第一条（施行期日）
Supplementary Provisions, Article 1Effective Date
This Ministerial Order comes into effect as of the day of enforcement of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (hereinafter referred to as the "Number Use Act") (October 5, 2015); provided, however, that the provisions set out in the following items come into effect as of the days specified respectively in those items:
この省令は、行政手続における特定の個人を識別するための番号の利用等に関する法律（以下「番号利用法」という。）の施行の日（平成二十七年十月五日）から施行する。ただし、次の各号に掲げる規定は、当該各号に定める日から施行する。
the provisions of Article 6, Articles 8 through 10, Article 12, Article 13, Article 15, Article 17, Articles 19 through 29, and Articles 31 through 38: the day of enforcement of the provisions set out in Article 1, item (iv) of the Supplementary Provisions of the Number Use Act (January 1, 2016).
第六条、第八条から第十条まで、第十二条、第十三条、第十五条、第十七条、第十九条から第二十九条まで及び第三十一条から第三十八条までの規定 番号利用法附則第一条第四号に掲げる規定の施行の日（平成二十八年一月一日）
第十一条（雇用保険法施行規則の一部改正に伴う経過措置）
Supplementary Provisions, Article 11Transitional Measures Accompanying the Partial Amendment of the Ordinance for Enforcement of the Employment Insurance Act
Documents that are actually submitted or issued at the time of enforcement of this Ministerial Order and are used in accordance with the forms of the Ordinance for Enforcement of the Employment Insurance Act before amendment by the provisions of Article 27 (referred to as the "Former Forms" in the following paragraph) are deemed to be in accordance with the forms of the Ordinance for Enforcement of the Employment Insurance Act as amended by the provisions of that Article.
この省令の施行の際現に提出又は交付されている第二十七条の規定による改正前の雇用保険法施行規則の様式（次項において「旧様式」という。）により使用されている書類は、同条の規定による改正後の雇用保険法施行規則の様式によるものとみなす。
Paper forms in accordance with the Former Forms that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧様式による用紙については、当分の間、これを取り繕って使用することができる。
略称規定附則第一条第一項
この省令は、行政手続における特定の個人を識別するための番号の利用等に関する法律（以下「番号利用法」という。）の施行の日（平成二十七年十月五日）から施行する。ただし、次の各号に掲げる規定は、当該各号に定める日から施行する。
わかりやすく言うと
行政手続における特定の個人を識別するための番号の利用等に関する法律のことで、平成二十七年厚生労働省令第百五十号の附則で使われる略称です。
略称規定附則第十一条第一項・この規定及び附則第十一条第二項の中だけ
この省令の施行の際現に提出又は交付されている第二十七条の規定による改正前の雇用保険法施行規則の様式（次項において「旧様式」という。）により使用されている書類は、同条の規定による改正後の雇用保険法施行規則の様式によるものとみなす。
わかりやすく言うと
平成二十七年厚生労働省令第百五十号の第二十七条による改正前の雇用保険法施行規則の様式のことです。
