Supplementary Provisions, Article 6Transitional Measures Accompanying the Partial Amendment of the Ordinance for Enforcement of the Employment Insurance Act

①With regard to the notification of ceasing to be an insured person under Article 7, paragraph (2) for a person employed as a mariner who was born on or before April 1, 1959, the phrase "59 years of age" in that paragraph is deemed to be replaced with "54 years of age".

②With regard to the effect of the mariner's unemployment insurance certificate prescribed in Article 40-2, paragraph (1) of the former Ordinance for Enforcement of the Mariners Insurance Act that is actually issued at the time of enforcement of this Ministerial Order, the provisions then in force continue to govern.

③With regard to the application of Article 19 where a person who is a mariner intends to receive payment of the basic allowance for the first time on or after the Effective Date, the phrase "attached" in paragraph (1) of that Article is deemed to be replaced with "and the mariner's unemployment insurance certificate prescribed in Article 48-2, paragraph (1) of the Ordinance for Enforcement of the Mariners Insurance Act (Order of the Ministry of Health and Welfare No. 5 of 1940) before amendment attached".

⑤The employment insurance notification of acquisition of insured status under Form No. 2 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "former Employment Insurance Ordinance"), the employment insurance notification of loss of insured status and employment insurance notification of change of name under Form No. 4 of the former Employment Insurance Ordinance, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the former Employment Insurance Ordinance, the notification of taking public vocational training, etc. and notification of commuting to public vocational training, etc. under Form No. 12 of the former Employment Insurance Ordinance, the written declaration for recognition of unemployment under Form No. 14 of the former Employment Insurance Ordinance, the certificate of attendance at public vocational training, etc. under Form No. 15 of the former Employment Insurance Ordinance, the written application for extension of the benefit period and written application for extension of the eligibility period for educational training benefits under Form No. 16 of the former Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment and notification of change of the desired financial institution for payment under Form No. 18 of the former Employment Insurance Ordinance, the qualified recipient's notification of change of name and qualified recipient's notification of change of address under Form No. 20 of the former Employment Insurance Ordinance, the written application for injury and disease allowance under Form No. 22 of the former Employment Insurance Ordinance, the elderly qualified recipient's written declaration for recognition of unemployment under Form No. 22-3 of the former Employment Insurance Ordinance, the specially qualified recipient's written declaration for recognition of unemployment under Form No. 24 of the former Employment Insurance Ordinance, the written application for employment allowance under Form No. 29 of the former Employment Insurance Ordinance, the written application for re-employment allowance under Form No. 29-2 of the former Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the former Employment Insurance Ordinance, the written application for relocation expenses under Form No. 30 of the former Employment Insurance Ordinance, the certificate of relocation under Form No. 32 of the former Employment Insurance Ordinance, the written application for wide-area job-seeking activity expenses under Form No. 33 of the former Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the former Employment Insurance Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance notification of acquisition of insured status under Form No. 2 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "new Employment Insurance Ordinance"), the employment insurance notification of loss of insured status and employment insurance notification of change of name under Form No. 4 of the new Employment Insurance Ordinance, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the new Employment Insurance Ordinance, the notification of taking public vocational training, etc. and notification of commuting to public vocational training, etc. under Form No. 12 of the new Employment Insurance Ordinance, the written declaration for recognition of unemployment under Form No. 14 of the new Employment Insurance Ordinance, the certificate of attendance at public vocational training, etc. under Form No. 15 of the new Employment Insurance Ordinance, the written application for extension of the benefit period and written application for extension of the eligibility period for educational training benefits under Form No. 16 of the new Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment and ⑥The employment insurance separation slip under Form No. 6 of the former Employment Insurance Ordinance, the employment insurance insured person's certificate under Form No. 7 of the former Employment Insurance Ordinance, the employment insurance qualified recipient's certificate under Form No. 11 of the former Employment Insurance Ordinance, the employment insurance elderly qualified recipient's certificate under Form No. 11-2 of the former Employment Insurance Ordinance, the employment insurance specially qualified recipient's certificate under Form No. 11-3 of the former Employment Insurance Ordinance, the insurance record book for insured day workers under Form No. 11-4 of the former Employment Insurance Ordinance, and the written decision on payment of relocation expenses under Form No. 31 of the former Employment Insurance Ordinance that are actually issued at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance separation slip under Form No. 6 of the new Employment Insurance Ordinance, the employment insurance insured person's certificate under Form No. 7 of the new Employment Insurance Ordinance, the employment insurance qualified recipient's certificate under Form No. 11 of the new Employment Insurance Ordinance, the employment insurance elderly qualified recipient's certificate under Form No. 11-2 of the new Employment Insurance Ordinance, the employment insurance specially qualified recipient's certificate under Form No. 11-3 of the new Employment Insurance Ordinance, the insurance record book for insured day workers under Form No. 11-4 of the new Employment Insurance Ordinance, and the written decision on payment of relocation expenses under Form No. 31 of the new Employment Insurance Ordinance.
