Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of the date of enforcement of the provisions listed in Article 1, item (ii) of the Supplementary Provisions of the Act Partially Amending the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures, etc. (hereinafter referred to as the "Amending Act") (December 2, 2024).
この省令は、行政手続における特定の個人を識別するための番号の利用等に関する法律等の一部を改正する法律（以下「改正法」という。）附則第一条第二号に掲げる規定の施行の日（令和六年十二月二日）から施行する。

第十四条（雇用保険法施行規則の一部改正に伴う経過措置）
With regard to the insured person's certificate of health insurance in the case where a person who, at the time of enforcement of this Ministerial Order, has actually been issued an insured person's certificate by the Association or a health insurance society submits, on or after the Effective Date, an employment insurance written application for reissuance of an insured person's certificate under Article 10, paragraph (3) of the Ordinance for Enforcement of the Employment Insurance Act, the provisions then in force continue to govern during the period for which that insured person's certificate was to remain effective.
この省令の施行の際現に協会又は健康保険組合から被保険者証の交付を受けている者が、施行日以後に雇用保険法施行規則第十条第三項の規定に基づき雇用保険被保険者証再交付申請書を提出する場合における当該健康保険の被保険者証については、当該被保険者証が効力を有するとされた間は、なお従前の例による。

第十五条
With regard to the insured person's certificate of National Health Insurance in the case where a person who, at the time of enforcement of this Ministerial Order, has actually been issued an insured person's certificate of National Health Insurance by a municipality or a National Health Insurance Society submits, on or after the Effective Date, a notification of acquisition of insured status as an insured day worker under Article 71, paragraph (2) of the Ordinance for Enforcement of the Employment Insurance Act, the provisions then in force continue to govern until the validity period of that insured person's certificate has elapsed.
この省令の施行の際現に市町村又は国民健康保険組合から国民健康保険の被保険者証の交付を受けている者が、施行日以後に雇用保険法施行規則第七十一条第二項の規定に基づき日雇労働被保険者資格取得届を提出する場合における当該国民健康保険の被保険者証については、当該被保険者証の有効期間が経過するまでの間は、なお従前の例による。

第十六条（様式に関する経過措置）
Documents that actually exist at the time of enforcement of this Ministerial Order and are used in accordance with the forms before amendment by this Ministerial Order (limited to the notification of acquisition of insured status under health insurance in accordance with Form No. 3 and Form No. 3-2 of the Ordinance for Enforcement of the Health Insurance Act, the notification of change of name of an insured person of health insurance in accordance with Form No. 7 of that Ordinance, the notification of loss of insured status under health insurance in accordance with Form No. 8 and Form No. 8-2 of that Ordinance, the elderly recipient's certificate in accordance with Form No. 10 of that Ordinance, the certificate for receiving treatment for specified diseases in accordance with Form No. 13 of that Ordinance, the certificate of authorization for application of the limit amount in accordance with Form No. 13-2 of that Ordinance, and the certificate of authorization for application of the limit amount and reduction of the standard amount to be borne in accordance with Form No. 14 of that Ordinance; the elderly recipient's certificate in accordance with Form No. 2 of the Ordinance for Enforcement of the Mariners Insurance Act, the certificate for receiving treatment for specified diseases in accordance with Form No. 5 of that Ordinance, the certificate of authorization for application of the limit amount in accordance with Form No. 6 of that Ordinance, and the certificate of authorization for application of the limit amount and reduction of the standard amount to be borne in accordance with Form No. 7 of that Ordinance; the National Health Insurance inspection certificate in accordance with Form No. 5 and Form No. 6 of the Ordinance for Enforcement of the National Health Insurance Act; the late-stage elderly medical care certificate for receiving treatment for specified diseases in accordance with Form No. 5 of the Ordinance for Enforcement of the Act on Assurance of Medical Care for Elderly People, and the late-stage elderly medical care inspection certificate in accordance with Form No. 7, Form No. 8 and Form No. 10 of that Ordinance; the employment insurance separation slip in accordance with Form No. 6 (2) of the Ordinance for Enforcement of the Employment Insurance Act, the written application for educational training benefits in accordance with Form No. 33-2 of that Ordinance, and the educational training benefits and educational training support benefits qualification confirmation form in accordance with Form No. 33-2-2 of that Ordinance; referred to as the "Former Forms" in the following paragraph) are deemed to be in accordance with the forms as amended by this Ministerial Order.
