附 則
This Ministerial Order comes into effect as of April 1, 2025; provided, however, that the amending provisions to Form No. 33-2 (page 2), Form No. 33-2-2 (page 2), Form No. 33-2-4 (page 2) and Form No. 33-2-5 (page 2) come into effect as of the date of promulgation.
この省令は、令和七年四月一日から施行する。ただし、様式第三十三号の二（第二面）、様式第三十三号の二の二（第二面）、様式第三十三号の二の四（第二面）及び様式第三十三号の二の五（第二面）の改正規定は、公布の日から施行する。
Documents that were actually submitted or issued at the time of enforcement of this Ministerial Order and are used in accordance with the forms before amendment by this Ministerial Order (referred to as the "Former Forms" in the following paragraph) are deemed to be in accordance with the forms as amended by this Ministerial Order.
この省令の施行の際現に提出され又は交付されているこの省令による改正前の様式（次項において「旧様式」という。）により使用されている書類は、この省令による改正後の様式によるものとみなす。
Paper forms in accordance with the Former Forms that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧様式による用紙については、当分の間、これを取り繕って使用することができる。
