附　則

This Ministerial Order comes into effect as of January 1, 2011.

この省令は、平成二十三年一月一日から施行する。

(hereinafter referred to as the "former Employment Insurance Ordinance"), the employment insurance notification of loss of insured status and employment insurance notification of change of name under Form No. 4 of the former Employment Insurance Ordinance, the employment insurance written application for reissuance of an insured person's certificate under Form No. 8 of the former Employment Insurance Ordinance, the employment insurance notification of transfer under Form No. 10 of the former Employment Insurance Ordinance, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the former Employment Insurance Ordinance, the notification of taking public vocational training, etc. and notification of commuting to public vocational training, etc. under Form No. 12 of the former Employment Insurance Ordinance, the written declaration for recognition of unemployment under Form No. 14 of the former Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment and notification of change of the desired financial institution for payment under Form No. 18 of the former Employment Insurance Ordinance, the qualified recipient's notification of change of name and qualified recipient's notification of change of address under Form No. 20 of the former Employment Insurance Ordinance, the written application for injury and disease allowance under Form No. 22 of the former Employment Insurance Ordinance, the written application for employment allowance under Form No. 29 of the former Employment application under Form No. 33-5-2 of the former Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the former Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the former Employment Insurance Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance notification of acquisition of insured status under Form No. 2 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "new Employment Insurance Ordinance"), the employment insurance notification of loss of insured status and employment insurance notification of change of name under Form No. 4 of the new Employment Insurance Ordinance, the employment insurance written application for reissuance of an insured person's certificate under Form No. 8 of the new Employment Insurance Ordinance, the employment insurance notification of transfer under Form No. 10 of the new Employment Insurance Ordinance, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the new Employment Insurance Ordinance, the notification of taking public vocational training, etc. and notification of commuting to public vocational training, etc. under Form No. 12 of the new Employment Insurance Ordinance, the written declaration for recognition of unemployment under Form No. 14 of the new Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment and notification of change of the desired financial institution for payment under Form No. 18 of the new Employment Insurance Ordinance, the qualified recipient's notification of change of name and qualified recipient's notification of change of address under Form No. 20 of the new Employment Insurance Ordinance, the written application for injury and disease allowance under Form No. 22 of the new Employment Insurance Ordinance, the written application for employment allowance under Form No. 29 of the new Employment Insurance Ordinance, the written application for re-employment allowance under Form No. 29-2 of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the new Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the new Employment Insurance Ordinance, the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-5 of the new Employment Insurance Ordinance, the childcare leave benefits application under Form No. 33-5-2 of the new Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the new Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status summary sheet for submission by optical disc or similar medium under Form No. 35 of the new Employment Insurance Ordinance.

この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧雇保則」という。）様式第二号による雇用保険被保険者資格取得届、旧雇保則様式第四号による雇用保険被保険者資格喪失届及び雇用保険被保険者氏名変更届、旧雇保則様式第八号による雇用保険被保険者証再交付申請書、旧雇保則様式第十号による雇用保険被保険者転勤届、旧雇保則様式第十号の四による未支給失業等給付請求書、旧雇保則様式第十二号による公共職業訓練等受講届及び公共職業訓練等通所届、旧雇保則様式第十四号による失業認定申告書、旧雇保則様式第十八号による払渡希望金融機関指定届及び払渡希望金融機関変更届、旧雇保則様式第二十号による受給資格者氏名変更届及び受給資格者住所変更届、旧雇保則様式第二十二号による傷病手当支給申請書、旧雇保則様式第二十九号による就業手当支給申請書、旧雇保則様式第二十九号の二による再就職手当支給申請書、旧雇保則様式第二十九号の三による常用就職支度手当支給申請書、旧雇保則様式第三十三号の二による教育訓練給付金支給申請書、旧雇保則様式第三十三号の三による高年齢雇用継続給付受給資格確認票・（初回）高年齢雇用継続給付支給申請書、旧雇保則様式第三十三号の三の二による高年齢雇用継続給付支給申請書、旧雇保則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書、旧雇保則様式第三十三号の五の二による育児休業給付金支給申請書、旧雇保則様式第三十三号の六による介護休業給付金支給申請書並びに旧雇保則様式第三十五号による雇用保険被保険者資格取得届（新規）フレキシブルディスク提出用総括票は、それぞれこの省令による改正後の雇用保険法施行規則（以下「新雇保則」という。）様式第二号による雇用保険被保険者資格取得届、新雇保則様式第四号による雇用保険被保険者資格喪失届及び雇用保険被保険者氏名変更届、新雇保則様式第八号による雇用保険被保険者証再交付申請書、新雇保則様式第十号による雇用保険被保険者転勤届、新雇保則様式第十号の四による未支給失業等給付請求書、新雇保則様式第十二号による公共職業訓練等受講届及び公共職業訓練等通所届、新雇保則様式第十四号による失業認定申告書、新雇保則様式第十八号による払渡希望金融機関指定届及び払渡希望金融機関変更届、新雇保則様式第二十号による受給資格者氏名変更届及び受給資格者住所変更届、新雇保則様式第二十二号による傷病手当支給申請書、新雇保則様式第二十九号による就業手当支給申請書、新雇保則様式第二十九号の二による再就職手当支給申請書、新雇保則様式第二十九号の三による常用就職支度手当支給申請書、新雇保則様式第三十三号の二による教育訓練給付金支給申請書、新雇保則様式第三十三号の三による高年齢雇用継続給付受給資格確認票・（初回）高年齢雇用継続給付支給申請書、新雇保則様式第三十三号の三の二による高年齢雇用継続給付支給申請書、新雇保則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書、新雇保則様式第三十三号の五の二による育児休業給付金支給申請書、新雇保則様式第三十三号の六による介護休業給付金支給申請書並びに新雇保則様式第三十五号による雇用保険被保険者資格取得届光ディスク等提出用総括票とみなす。

The employment insurance separation slip under Form No. 6 of the former Employment Insurance Ordinance, the employment insurance qualified recipient's certificate under Form No. 11 of the former Employment Insurance Ordinance, the employment insurance elderly qualified recipient's certificate under Form No. 11-2 of the former Employment Insurance Ordinance, and the employment insurance specially qualified recipient's certificate under Form No. 11-3 of the former Employment Insurance Ordinance that are actually issued at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance separation slip under Form No. 6 of the new Employment Insurance Ordinance, the employment insurance qualified recipient's certificate under Form No. 11 of the new Employment Insurance Ordinance, the employment insurance elderly qualified recipient's certificate under Form No. 11-2 of the new Employment Insurance Ordinance, and the employment insurance specially qualified recipient's certificate under Form No. 11-3 of the new Employment Insurance Ordinance.

この省令の施行の際現に交付されている旧雇保則様式第六号による雇用保険被保険者離職票、旧雇保則様式第十一号による雇用保険受給資格者証、旧雇保則様式第十一号の二による雇用保険高年齢受給資格者証及び旧雇保則様式第十一号の三による雇用保険特例受給資格者証は、それぞれ新雇保則様式第六号による雇用保険被保険者離職票、新雇保則様式第十一号による雇用保険受給資格者証、新雇保則様式第十一号の二による雇用保険高年齢受給資格者証並びに新雇保則様式第十一号の三による雇用保険特例受給資格者証とみなす。

The employment insurance notification of acquisition of insured status under Article 6, paragraph (1) of the new Employment Insurance Ordinance, the employment insurance notification of loss of insured status under Article 7, paragraph (1) of the new Employment Insurance Ordinance, the employment insurance written application for reissuance of an insured person's certificate under Article 10, paragraph (3) of the new Employment Insurance Ordinance, the employment insurance notification of transfer under Article 13, paragraph (1) of the new Employment Insurance Ordinance, the employment insurance notification of change of name under Article 14, paragraph (1) of the new Employment Insurance Ordinance, the written claim for unpaid benefits for unemployment, etc. under Article 17-2, paragraph (1) of the new Employment Insurance Ordinance, the employment insurance qualified recipient's certificate under item (i) of that paragraph, the employment insurance elderly qualified recipient's certificate under item (ii) of that paragraph, the employment insurance specially qualified recipient's certificate under item (iii) of that paragraph, the notification of taking public vocational training, etc. and notification of commuting to public vocational training, etc. under Article 21, paragraph (1) of the new Employment Insurance Ordinance, the written declaration for recognition of unemployment under Article 22, paragraph (1) of the new Employment Insurance Ordinance, the certificate of attendance at public vocational training, etc. under Article 27, paragraph (1) of the new Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment under Article 45, paragraph (2) of the new Employment Insurance Ordinance, the notification of change of the desired financial institution for payment under paragraph (3) of that Article, the qualified recipient's notification of change of name and qualified recipient's notification of change of address under Article 49, paragraph (1) of the new Employment Insurance Ordinance, the written application for injury and disease allowance under Article 63, paragraph (2) of the new Employment Insurance Ordinance, the written application for employment allowance under Article 82-5, paragraph (1) of the new Employment Insurance Ordinance, the written application for re-employment allowance under Article 82-7, paragraph (1) of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Article 84, paragraph (1) of the new Employment Insurance Ordinance, the written application for educational training benefits under Article 101-2-8, paragraph (1) of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly and written application for continuous employment benefits for the elderly under Article 101-5, paragraph (1) of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application and childcare leave benefits application under Article 101-13, paragraph (1) of the new Employment Insurance Ordinance, the application for nursing care leave benefits under Article 101-19, paragraph (1) of the new Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status summary sheet for submission by optical disc or similar medium under Article 146, paragraph (1), item (i) of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.

新雇保則第六条第一項の雇用保険被保険者資格取得届、新雇保則第七条第一項の雇用保険被保険者資格喪失届、新雇保則第十条第三項の雇用保険被保険者証再交付申請書、新雇保則第十三条第一項の雇用保険被保険者転勤届、新雇保則第十四条第一項の雇用保険被保険者氏名変更届、新雇保則第十七条の二第一項の未支給失業等給付請求書、同項第一号の雇用保険受給資格者証、同項第二号の雇用保険高年齢受給資格者証、同項第三号の雇用保険特例受給資格者証、新雇保則第二十一条第一項の公共職業訓練等受講届及び公共職業訓練等通所届、新雇保則第二十二条第一項の失業認定申告書、新雇保則第二十七条第一項の公共職業訓練等受講証明書、新雇保則第四十五条第二項の払渡希望金融機関指定届、同条第三項の払渡希望金融機関変更届、新雇保則第四十九条第一項の受給資格者氏名変更届及び受給資格者住所変更届、新雇保則第六十三条第二項の傷病手当支給申請書、新雇保則第八十二条の五第一項の就業手当支給申請書、新雇保則第八十二条の七第一項の再就職手当支給申請書、新雇保則第八十四条第一項の常用就職支度手当支給申請書、新雇保則第百一条の二の八第一項の教育訓練給付金支給申請書、新雇保則第百一条の五第一項の高年齢雇用継続給付受給資格確認票・（初回）高年齢雇用継続給付支給申請書及び高年齢雇用継続給付支給申請書、新雇保則第百一条の十三第一項の育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書及び育児休業給付金支給申請書、新雇保則第百一条の十九第一項の介護休業給付金支給申請書並びに新雇保則第百四十六条第一項第一号の雇用保険被保険者資格取得届光ディスク等提出用総括票は、当分の間、なお旧雇保則の相当様式によることができる。
