Supplementary Provisions

This Ministerial Order comes into effect as of July 1, 2014; provided, however, that the amending provision for Form No. 10-4 and the amending provision for Form No. 33-2 (excluding the portion adding ", in principle," after "the applicant themselves") in Article 1 come into effect as of October 1, 2014.

The written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the Ordinance for Enforcement of the Employment Insurance Act as amended by the provisions of Article 1 (hereinafter in this Article referred to as the "new Employment Insurance Ordinance"), the employment insurance qualified recipient's certificate under Form No. 11 of the new Employment Insurance Ordinance, the notification of designation of the desired financial institution for payment and the notification of change of the desired financial institution for payment under Form No. 18 of the new Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-5 of the new Employment Insurance Ordinance, and the summary sheet for submission of the employment insurance notification of transfer on an optical disc or similar medium under Form No. 37 of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the Ordinance for Enforcement of the Employment Insurance Act before amendment by the provisions of Article 1.
