Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of the date of enforcement of the Act Partially Amending the Act on Childcare Leave, Caregiver Leave, and Other Measures for the Welfare of Workers Caring for Children or Other Family Members and the Employment Insurance Act (Act No. 65 of 2009; hereinafter referred to as the "Amending Act") (June 30, 2010).
この省令は、育児休業、介護休業等育児又は家族介護を行う労働者の福祉に関する法律及び雇用保険法の一部を改正する法律（平成二十一年法律第六十五号。以下「改正法」という。）の施行の日（平成二十二年六月三十日）から施行する。

第三条（雇用保険法施行規則の一部改正に伴う経過措置）
The employment insurance certificate of wages at the commencement of leave / certificate of wages at the commencement of shortening working hours under Form No. 10-2, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-5, and the childcare leave benefits application under Form No. 33-5-2, all of the Ordinance for Enforcement of the Employment Insurance Act before amendment by the provisions of Article 4 (hereinafter referred to as the "former Employment Insurance Ordinance"), that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance certificate of wages at the commencement of leave / certificate of wages at the commencement of shortening prescribed working hours under Form No. 10-2, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-5, and the childcare leave benefits application under Form No. 33-5-2, all of the Ordinance for Enforcement of the Employment Insurance Act as amended by the provisions of Article 4 (hereinafter referred to as the "new Employment Insurance Ordinance").
この省令の施行の際現に提出されている第四条の規定による改正前の雇用保険法施行規則（以下「旧雇保則」という。）様式第十号の二による雇用保険被保険者休業・勤務時間短縮開始時賃金証明書、旧雇保則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書並びに旧雇保則様式第三十三号の五の二による育児休業給付金支給申請書は、それぞれ、第四条の規定による改正後の雇用保険法施行規則（以下「新雇保則」という。）様式第十号の二による雇用保険被保険者休業・所定労働時間短縮開始時賃金証明書、新雇保則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書並びに新雇保則様式第三十三号の五の二による育児休業給付金支給申請書とみなす。
The employment insurance certificate slip of wages at the commencement of leave / certificate slip of wages at the commencement of shortening working hours under Form No. 10-3 of the former Employment Insurance Ordinance that is actually issued at the time of enforcement of this Ministerial Order is deemed to be the employment insurance certificate slip of wages at the commencement of leave / certificate slip of wages at the commencement of shortening prescribed working hours under Form No. 10-3 of the new Employment Insurance Ordinance.
この省令の施行の際現に交付されている旧雇保則様式第十号の三による雇用保険被保険者休業・勤務時間短縮開始時賃金証明票は、新雇保則様式第十号の三による雇用保険被保険者休業・所定労働時間短縮開始時賃金証明票とみなす。
The employment insurance certificate of wages at the commencement of leave / certificate of wages at the commencement of shortening prescribed working hours under Article 14-4, paragraph (1) of the new Employment Insurance Ordinance, the employment insurance certificate slip of wages at the commencement of leave / certificate slip of wages at the commencement of shortening prescribed working hours under paragraph (3) of that Article, and the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application and the childcare leave benefits application under Article 101-13 of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
新雇保則第十四条の四第一項の雇用保険被保険者休業・所定労働時間短縮開始時賃金証明書、同条第三項の雇用保険被保険者休業・所定労働時間短縮開始時賃金証明票、新雇保則第百一条の十三の育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書及び育児休業給付金支給申請書は、当分の間、なお旧雇保則の相当様式によることができる。
