Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of November 28, 2011.
この省令は、平成二十三年十一月二十八日から施行する。

第二条（経過措置）
Form No. 4 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "former Employment Insurance Ordinance"), Form No. 5 of the former Employment Insurance Ordinance, or Form No. 10-2 of the former Employment Insurance Ordinance that is actually submitted at the time of enforcement of this Ministerial Order is deemed to be Form No. 4 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "new Employment Insurance Ordinance"), Form No. 5 of the new Employment Insurance Ordinance, or Form No. 10-2 of the new Employment Insurance Ordinance, respectively.
この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧雇保則」という。）様式第四号、旧雇保則様式第五号又は旧雇保則様式第十号の二は、それぞれこの省令による改正後の雇用保険法施行規則（以下「新雇保則」という。）様式第四号、新雇保則様式第五号又は新雇保則様式第十号の二とみなす。
Form No. 4 and Form No. 5 prescribed in Article 7, paragraph (1) of the new Employment Insurance Ordinance and Form No. 10-2 prescribed in Article 14-2 and Article 14-4 of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
新雇保則第七条第一項に規定する様式第四号、様式第五号並びに新雇保則第十四条の二及び第十四条の四に規定する様式第十号の二は、当分の間、なお旧雇保則の相当様式によることができる。
