附 則
This Ministerial Order comes into effect as of April 1, 2005.
この省令は、平成十七年四月一日から施行する。
The employment insurance certificate of wages at the commencement of leave under Form No. 10-2 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "Former Ordinance"), the employment insurance certificate slip of wages at the commencement of leave under Form No. 10-3 of the Former Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the Former Ordinance, the written application for basic childcare leave benefits under Form No. 33-5-2 of the Former Ordinance, and the application for nursing care leave benefits under Form No. 33-6 of the Former Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance certificate of wages at the commencement of leave under Form No. 10-2 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "New Ordinance"), the employment insurance certificate slip of wages at the commencement of leave under Form No. 10-3 of the New Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the New Ordinance, the written application for basic childcare leave benefits under Form No. 33-5-2 of the New Ordinance, and the application for nursing care leave benefits under Form No. 33-6 of the New Ordinance.
この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧規則」という。）様式第十号の二による雇用保険被保険者休業開始時賃金証明書、旧規則様式第十号の三による雇用保険被保険者休業開始時賃金証明票、旧規則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業基本給付金支給申請書、旧規則様式第三十三号の五の二による育児休業基本給付金支給申請書及び旧規則様式第三十三号の六による介護休業給付金支給申請書は、それぞれ、この省令による改正後の雇用保険法施行規則（以下「新規則」という。）様式第十号の二による雇用保険被保険者休業開始時賃金証明書、新規則第十号の三による雇用保険被保険者休業開始時賃金証明票、新規則様式第三十三号の五による育児休業給付受給資格確認票・（初回）育児休業基本給付金支給申請書、新規則様式第三十三号の五の二による育児休業基本給付金支給申請書及び新規則様式第三十三号の六による介護休業給付金支給申請書とみなす。
The employment insurance certificate of wages at the commencement of leave under Article 14-2, paragraph (1) of the New Ordinance, the employment insurance certificate slip of wages at the commencement of leave under Article 14-2, paragraph (2) of the New Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits and the written application for basic childcare leave benefits under Article 101-13, paragraph (1) of the New Ordinance, and the application for nursing care leave benefits under Article 101-19, paragraph (1) of the New Ordinance may, for the time being, continue to be in the corresponding forms of the Former Ordinance.
新規則第十四条の二第一項の雇用保険被保険者休業開始時賃金証明書、新規則第十四条の二第二項の雇用保険被保険者休業開始時賃金証明票、新規則第百一条の十三第一項の育児休業給付受給資格確認票・（初回）育児休業基本給付金支給申請書及び育児休業基本給付金支給申請書並びに新規則第百一条の十九第一項の介護休業給付金支給申請書は、当分の間、なお旧規則の相当様式によることができる。
