(hereinafter in this Article referred to as the "former Employment Insurance Ordinance"), the written application for re-employment allowance under Form No. 29-2 of the former Employment Insurance Ordinance, the written application for employment promotion settlement allowance under Form No. 29-2-2 of the former Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the former Employment Insurance Ordinance, the written application for relocation expenses under Form No. 30 of the former Employment Insurance Ordinance, the written application for wide-area job-seeking activity expenses under Form No. 33 of the former Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the former Employment Insurance Ordinance, the educational training benefits (relating to Article 101-2-7, item (ii)) and educational training support benefits qualification confirmation form under Form No. 33-2-2 of the former Employment Insurance Ordinance, the written application for educational training benefits (relating to Article 101-2-7, item (ii)) under Form No. 33-2-4 of the former Employment Insurance Ordinance, the written application for educational training benefits (relating to Article 101-2-7, item (iii)) under Form No. 33-2-5 of the former Employment Insurance Ordinance, the certificate of attendance for educational training support benefits under Form No. 33-2-7 of the former Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the former Employment Insurance Ordinance, the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the former Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-5 of the former Employment Insurance Ordinance, the childcare leave benefits application under Form No. 33-5-2 of the former Employment Insurance Ordinance, and the application for nursing care leave benefits under Form No. 33-6 of the former Employment Insurance Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter in this Article referred to as the "new Employment Insurance Ordinance"), the written application for re-employment allowance under Form No. 29-2 of the new Employment Insurance Ordinance, the written application for employment promotion settlement allowance under Form No. 29-2-2 of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the new Employment Insurance Ordinance, the written application for relocation expenses under Form No. 30 of the new Employment Insurance Ordinance, the written application for wide-area job-seeking activity expenses under Form No. 33 of the new Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the new Employment Insurance Ordinance, the educational training benefits (relating to Article 101-2-7, item (ii)) and educational training support benefits qualification confirmation form under Form No. 33-2-2 of the new Employment Insurance Ordinance, the written application for educational training benefits (relating to Article 101-2-7, item (ii)) under Form No. 33-2-4 of the new Employment Insurance Ordinance, the written application for educational training benefits (relating to Article 101-2-7, item (iii)) under Form No. 33-2-5 of the new Employment Insurance Ordinance, the certificate of attendance for educational training support benefits under Form No. 33-2-7 of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the new Employment Insurance ②The written notice of extension of the benefit period and the written notice of extension of the eligibility period for educational training benefits under Form No. 17 of the former Employment Insurance Ordinance that are actually issued at the time of enforcement of this Ministerial Order are deemed to be the written notice of extension of the benefit period and the written notice of extension of the eligibility period for educational training benefits under Form No. 17 of the new Employment Insurance Ordinance.

③The written claim for unpaid benefits for unemployment, etc. under Article 17-2, paragraph (1) of the new Employment Insurance Ordinance, the written notice of extension of the benefit period under Article 31, paragraph (4) of the new Employment Insurance Ordinance and the written notice of extension of the eligibility period for educational training benefits under Article 101-2-5, paragraph (3) of the new Employment Insurance Ordinance, the written application for re-employment allowance under Article 82-7, paragraph (1) of the new Employment Insurance Ordinance, the written application for employment promotion settlement allowance under Article 83-4, paragraph (1) of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Article 84, paragraph (1) of the new Employment Insurance Ordinance, the written application for relocation expenses under Article 92, paragraph (1) of the new Employment Insurance Ordinance, the written application for wide-area job-seeking activity expenses under Article 99, paragraph (1) of the new Employment Insurance Ordinance, the written application for educational training benefits under Article 101-2-11, paragraph (1), and Article 101-2-12, paragraphs (5) and (6) of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly and the written application for continuous employment benefits for the elderly under Article 101-5, paragraph (1) of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application and the childcare leave benefits application under Article 101-13, paragraph (1) of the new Employment Insurance Ordinance, the application for nursing care leave benefits under Article 101-19, paragraph (1) of the new Employment Insurance Ordinance, and the certificate of attendance for educational training support benefits under Article 28, paragraph (1) of the Supplementary Provisions of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
