Supplementary Provisions

This Ministerial Order comes into effect as of February 22, 2010.

①The childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "former Employment Insurance Ordinance") and the written application for basic childcare leave benefits / written application for return-to-work benefits for persons on childcare leave under Form No. 33-5-2 of the former Employment Insurance Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "new Employment Insurance Ordinance") and the written application for basic childcare leave benefits / written application for return-to-work benefits for persons on childcare leave under Form No. 33-5-2 of the new Employment Insurance Ordinance.

②The childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits and the written application for basic childcare leave benefits under Article 101-13, paragraph (1) of the new Employment Insurance Ordinance, and the written application for return-to-work benefits for persons on childcare leave under Article 101-14, paragraph (1) of the new Employment Insurance Ordinance, may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
