The employment insurance certificate of wages at the commencement of leave under Article 14-3, paragraph (1) of the New Ordinance, the written claim for unpaid benefits for unemployment, etc. under Article 17-2, paragraph (1) of the New Ordinance, the written declaration for recognition of unemployment under Article 22, paragraph (1) of the New Ordinance, the certificate of attendance at public vocational training, etc. under Article 27 of the New Ordinance, the notification of designation of the desired financial institution for payment under Article 45, paragraph (2) of the New Ordinance, the notification of change of the desired financial institution for payment under Article 45, paragraph (3) of the New Ordinance, the written application for injury and disease allowance under Article 63, paragraph (2) of the New Ordinance, the elderly qualified recipient's written declaration for recognition of unemployment under Article 65-5 of the New Ordinance, the specially qualified recipient's written declaration for recognition of unemployment under Article 69 of the New Ordinance, and the written application for educational training benefits under Article 101-2-8, paragraph (1) of the New Ordinance may, for the time being, continue to be in the corresponding forms of the Ordinance for Enforcement of the Employment Insurance Act before amendment by the provisions of Article 1.
