附　則

This Ministerial Order comes into effect as of June 14, 2026.

この省令は、令和八年六月十四日から施行する。

The employment insurance notification of acquisition of insured status in accordance with Form No. 2 and Form No. 2-2 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "former Employment Insurance Ordinance"), the employment insurance notification of loss of insured status in accordance with Form No. 4 and Form No. 4-2 of the former Employment Insurance Ordinance, the summary sheet for submission of the employment insurance notification of acquisition of insured status on an optical disc or similar medium in accordance with Form No. 35 of the former Employment Insurance Ordinance, the summary sheet for submission of the employment insurance notification of loss of insured status on an optical disc or similar medium in accordance with Form No. 36 of the former Employment Insurance Ordinance, and the summary sheet for submission of the employment insurance notification of transfer on an optical disc or similar medium in accordance with Form No. 37 of the former Employment Insurance Ordinance, which are actually submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, the employment insurance notification of acquisition of insured status prescribed in Article 6, paragraph (1) of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "new Employment Insurance Ordinance"), the employment insurance notification of loss of insured status prescribed in Article 7, paragraph (1) of the new Employment Insurance Ordinance, and the summary sheet prescribed in Article 146, paragraph (1) of the new Employment Insurance Ordinance.

この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧雇保則」という。）様式第二号及び様式第二号の二による雇用保険被保険者資格取得届、旧雇保則様式第四号及び様式第四号の二による雇用保険被保険者資格喪失届並びに旧雇保則様式第三十五号による雇用保険被保険者資格取得届光ディスク等提出用総括票、旧雇保則様式第三十六号による雇用保険被保険者資格喪失届光ディスク等提出用総括票及び旧雇保則様式第三十七号による雇用保険被保険者転勤届光ディスク等提出用総括票は、それぞれ、この省令による改正後の雇用保険法施行規則（以下「新雇保則」という。）第六条第一項に規定する雇用保険被保険者資格取得届、新雇保則第七条第一項に規定する雇用保険被保険者資格喪失届及び新雇保則第百四十六条第一項に規定する総括票とみなす。

Paper forms in accordance with the forms before amendment by this Ministerial Order that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.

この省令の施行の際現にあるこの省令による改正前の様式による用紙については、当分の間、これを取り繕って使用することができる。
