第二条（経過措置）
The written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "former Employment Insurance Ordinance"), the application for nursing care leave benefits under Form No. 33-6 of the former Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-7 of the former Employment Insurance Ordinance, and the childcare leave benefits application under Form No. 33-8 of the former Employment Insurance Ordinance, which have actually been submitted at the time this Ministerial Order comes into effect, are deemed to be, respectively, the written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter in this paragraph and paragraph (3) referred to as the "new Employment Insurance Ordinance"), the application for nursing care leave benefits prescribed in Article 101-19, paragraph (1) of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application prescribed in Article 101-30, paragraph (1) of the new Employment Insurance Ordinance, and the childcare leave benefits application prescribed in paragraph (4) of that Article.
この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧雇保則」という。）様式第十号の四による未支給失業等給付請求書、旧雇保則様式第三十三号の六による介護休業給付金支給申請書、旧雇保則様式第三十三号の七による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書及び旧雇保則様式第三十三号の八による育児休業給付金支給申請書は、それぞれ、この省令による改正後の雇用保険法施行規則（以下この項及び第三項において「新雇保則」という。）様式第十号の四による未支給失業等給付請求書、新雇保則第百一条の十九第一項に規定する介護休業給付金支給申請書、新雇保則第百一条の三十第一項に規定する育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書及び同条第四項に規定する育児休業給付金支給申請書とみなす。
The written claim for unpaid benefits for unemployment, etc. under Form No. 10-4 of the former Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the former Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) childcare leave benefits application under Form No. 33-7 of the former Employment Insurance Ordinance, and the childcare leave benefits application under Form No. 33-8 of the former Employment Insurance Ordinance that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧雇保則様式第十号の四による未支給失業等給付請求書、旧雇保則様式第三十三号の六による介護休業給付金支給申請書、旧雇保則様式第三十三号の七による育児休業給付受給資格確認票・（初回）育児休業給付金支給申請書及び旧雇保則様式第三十三号の八による育児休業給付金支給申請書については、当分の間、これを取り繕って使用することができる。
Where an insured person who received a notice under Article 101-30, paragraph (3) of the former Employment Insurance Ordinance before the day of enforcement of this Ministerial Order seeks to receive payment of childcare leave benefits on or after that day for the payment unit period pertaining to that notice, that notice is deemed to be a notice under Article 101-30, paragraph (2) of the new Employment Insurance Ordinance, and the provisions of paragraph (4) of that Article apply.
この省令の施行の日前に旧雇保則第百一条の三十第三項の規定による通知を受けた被保険者が、この省令の施行の日以後に当該通知に係る支給単位期間について育児休業給付金の支給を受けようとするときは、当該通知は新雇保則第百一条の三十第二項の規定による通知とみなして、同条第四項の規定を適用する。
