Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of the date of promulgation; provided, however, that the amending provision that adds one Article after Article 1-2 of the Supplementary Provisions (excluding the portion pertaining to Article 1-3, paragraph (1) of the Supplementary Provisions) comes into effect as of May 1, 2018, and the amending provisions to Form No. 10-2-2, Form No. 33-3, Form No. 33-3-2, Form No. 33-4, Form No. 33-5, Form No. 33-5-2, and Form No. 33-6 come into effect as of October 1 of the same year.
この省令は、公布の日から施行する。ただし、附則第一条の二の次に一条を加える改正規定（附則第一条の三第一項に係る部分を除く。）は平成三十年五月一日から、様式第十号の二の二、様式第三十三号の三、様式第三十三号の三の二、様式第三十三号の四、様式第三十三号の五、様式第三十三号の五の二及び様式第三十三号の六の改正規定は同年十月一日から施行する。
Supplementary Provisions, Article 2Transitional Measures
第二条（経過措置）
Documents that are actually submitted or issued at the time of enforcement of this Ministerial Order and are submitted in accordance with the forms (referred to as the "Former Forms" in the following paragraph) of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order are deemed to be in accordance with the forms of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order.
この省令の施行の際現に提出され、又は交付されているこの省令による改正前の雇用保険法施行規則の様式（次項において「旧様式」という。）により提出されている書類は、この省令による改正後の雇用保険法施行規則の様式によるものとみなす。
Paper forms in accordance with the Former Forms that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧様式による用紙については、当分の間、これを取り繕って使用することができる。
