第二条（経過措置）
The employment insurance notification of acquisition of insured status under Form No. 2 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "Former Ordinance"), the employment insurance separation certificate under Form No. 5 of the Former Ordinance, the employment insurance notification of change of classification under Form No. 9 of the Former Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the Former Ordinance that are actually submitted at the time of enforcement of this Ministerial Order, and the employment insurance separation slip under Form No. 6 of the Former Ordinance and the employment insurance insured person's certificate under Form No. 7 of the Former Ordinance that are actually issued at the time of enforcement of this Ministerial Order, are deemed to be, respectively, the employment insurance notification of acquisition of insured status under Form No. 2 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "New Ordinance"), the employment insurance separation certificate under Form No. 5 of the New Ordinance, the employment insurance notification of change of classification under Form No. 9 of the New Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the New Ordinance, and the employment insurance separation slip under Form No. 6 of the New Ordinance and the employment insurance insured person's certificate under Form No. 7 of the New Ordinance.
この省令の施行の際現に提出されているこの省令による改正前の雇用保険法施行規則（以下「旧規則」という。）様式第二号による雇用保険被保険者資格取得届、旧規則様式第五号による雇用保険被保険者離職証明書、旧規則様式第九号による雇用保険被保険者区分変更届及び旧規則様式第三十五号による雇用保険被保険者資格取得届（新規）フレキシブルディスク提出用総括票並びにこの省令の施行の際現に交付されている旧規則様式第六号による雇用保険被保険者離職票及び旧規則様式第七号による雇用保険被保険者証は、それぞれ、この省令による改正後の雇用保険法施行規則（以下「新規則」という。）様式第二号による雇用保険被保険者資格取得届、新規則様式第五号による雇用保険被保険者離職証明書、新規則様式第九号による雇用保険被保険者区分変更届及び新規則様式第三十五号による雇用保険被保険者資格取得届（新規）フレキシブルディスク提出用総括票並びに新規則様式第六号による雇用保険被保険者離職票及び新規則様式第七号による雇用保険被保険者証とみなす。
The employment insurance notification of acquisition of insured status under Article 6, paragraph (1) of the New Ordinance, the employment insurance separation certificate under Article 7, paragraph (1), item (i) of the New Ordinance, the employment insurance separation slip under Article 7, paragraph (2) of the New Ordinance, the employment insurance insured person's certificate under Article 10, paragraph (1) of the New Ordinance, the employment insurance notification of change of classification under Article 12-2 of the New Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Article 146, paragraph (1) of the New Ordinance may, for the time being, continue to be in the corresponding forms of the Former Ordinance.
新規則第六条第一項の雇用保険被保険者資格取得届、新規則第七条第一項第一号の雇用保険被保険者離職証明書、新規則第七条第二項の雇用保険被保険者離職票、新規則第十条第一項の雇用保険被保険者証、新規則第十二条の二の雇用保険被保険者区分変更届及び新規則第百四十六条第一項の雇用保険被保険者資格取得届（新規）フレキシブルディスク提出用総括票は、当分の間、なお旧規則の相当様式によることができる。
