Supplementary Provisions

①This Ministerial Order comes into effect as of November 29, 2004.

②The written application for continuous employment benefits for the elderly under Form No. 33-3 of the Ordinance for Enforcement of the Employment Insurance Act before amendment by this Ministerial Order (hereinafter referred to as the "Former Ordinance") and the written application for basic childcare leave benefits under Form No. 33-5 of the Former Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the Ordinance for Enforcement of the Employment Insurance Act as amended by this Ministerial Order (hereinafter referred to as the "New Ordinance") or the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the New Ordinance, and the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the New Ordinance or the written application for basic childcare leave benefits under Form No. 33-5-2 of the New Ordinance.

③The continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly and the written application for continuous employment benefits for the elderly under Article 101-5, paragraph (1) of the New Ordinance, and the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits and the written application for basic childcare leave benefits under Article 101-13, paragraph (1) of the New Ordinance, may, for the time being, continue to be in the corresponding forms of the Former Ordinance.
