第十条（様式に関する経過措置）
The employment insurance notification of loss of insured status and the employment insurance notification of change of name under Form No. 4 of the former Employment Insurance Ordinance are deemed to be the employment insurance notification of loss of insured status and the employment insurance notification of change of name under Form No. 4 of the new Employment Insurance Ordinance.
旧雇保則様式第四号による雇用保険被保険者資格喪失届及び雇用保険被保険者氏名変更届は、新雇保則様式第四号による雇用保険被保険者資格喪失届及び雇用保険被保険者氏名変更届とみなす。
The employment insurance separation slip under Form No. 6 of the former Employment Insurance Ordinance that is actually issued at the time of enforcement of this Ministerial Order is deemed to be the employment insurance separation slip under Form No. 6 of the new Employment Insurance Ordinance.
The employment insurance certificate of seizure of property of persons in arrears on repayments, etc. under Article 17-7 of the new Employment Insurance Ordinance and the employment insurance inspection certificate under Article 144 of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
