第一条（施行期日）
This Ministerial Order comes into effect as of July 1, 2014; provided, however, that the amending provision for Form No. 10-4 and the amending provision for Form No. 33-2 (excluding the portion adding ", in principle," after "the applicant themselves") in Article 1 come into effect as of October 1, 2014.
この省令は、平成二十六年七月一日から施行する。ただし、第一条中様式第十号の四の改正規定及び様式第三十三号の二の改正規定（「申請者本人が」の下に「、原則として」を加える部分を除く。）は、平成二十六年十月一日から施行する。
