Supplementary Provisions, Article 4Transitional Measures Concerning Forms

①The written application under Article 101-2-3, paragraph (2) of the former Employment Insurance Ordinance that is actually submitted at the time of enforcement of this Ministerial Order is deemed to be the written application for extension of the eligibility period for educational training benefits under Form No. 16 of the new Employment Insurance Ordinance.

②The employment insurance notification of acquisition of insured status under Form No. 2 of the former Employment Insurance Ordinance, the notification of taking public vocational training, etc. and the notification of commuting to public vocational training, etc. under Form No. 12 of the former Employment Insurance Ordinance, the written application for extension of the benefit period under Form No. 16 of the former Employment Insurance Ordinance, the written application for re-employment allowance under Form No. 29-2 of the former Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the former Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the former Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the former Employment Insurance Ordinance, the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the former Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the former Employment Insurance Ordinance, the written application for basic childcare leave benefits and the written application for return-to-work benefits for persons on childcare leave under Form No. 33-5-2 of the former Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the former Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the former Employment Insurance Ordinance that are actually submitted at the time of enforcement of this Ministerial Order are deemed to be, respectively, the employment insurance notification of acquisition of insured status under Form No. 2 of the new Employment Insurance Ordinance, the notification of taking public vocational training, etc. and the notification of commuting to public vocational training, etc. under Form No. 12 of the new Employment Insurance Ordinance, the written application for extension of the benefit period under Form No. 16 of the new Employment Insurance Ordinance, the written application for re-employment allowance under Form No. 29-2 of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the new Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the new Employment Insurance Ordinance, the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the new Employment Insurance Ordinance, the written application for basic childcare leave benefits and the written application for return-to-work benefits for persons on childcare leave under Form No. 33-5-2 of the new Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the new Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the new Employment Insurance Ordinance.

③The certificate of attendance at public vocational training, etc. under Form No. 15 of the former Employment Insurance Ordinance and the written notice of extension of the benefit period under Form No. 17 of the former Employment Insurance Ordinance that are actually issued at the time of enforcement of this Ministerial Order are deemed to be the certificate of attendance at public vocational training, etc. under Form No. 15 of the new Employment Insurance Ordinance and the written notice of extension of the benefit period under Form No. 17 of the new Employment Insurance Ordinance, respectively.

④The employment insurance notification of acquisition of insured status under Form No. 2 of the new Employment Insurance Ordinance, the notification of taking public vocational training, etc. and the notification of commuting to public vocational training, etc. under Form No. 12 of the new Employment Insurance Ordinance, the certificate of attendance at public vocational training, etc. under Form No. 15 of the new Employment Insurance Ordinance, the written application for extension of the benefit period and the written application for extension of the eligibility period for educational training benefits under Form No. 16 of the new Employment Insurance Ordinance, the written notice of extension of the benefit period under Form No. 17 of the new Employment Insurance Ordinance, the written application for re-employment allowance under Form No. 29-2 of the new Employment Insurance Ordinance, the written application for regular employment preparation allowance under Form No. 29-3 of the new Employment Insurance Ordinance, the written application for educational training benefits under Form No. 33-2 of the new Employment Insurance Ordinance, the continuous employment benefits for the elderly qualification confirmation form / (initial) written application for continuous employment benefits for the elderly under Form No. 33-3 of the new Employment Insurance Ordinance, the written application for continuous employment benefits for the elderly under Form No. 33-3-2 of the new Employment Insurance Ordinance, the childcare leave-related benefits qualification confirmation form / (initial) written application for basic childcare leave benefits under Form No. 33-5 of the new Employment Insurance Ordinance, the written application for basic childcare leave benefits and the written application for return-to-work benefits for persons on childcare leave under Form No. 33-5-2 of the new Employment Insurance Ordinance, the application for nursing care leave benefits under Form No. 33-6 of the new Employment Insurance Ordinance, and the employment insurance notification of acquisition of insured status (new) summary sheet for submission of flexible disks under Form No. 35 of the new Employment Insurance Ordinance may, for the time being, continue to be in the corresponding forms of the former Employment Insurance Ordinance.
