Supplementary Provisions, Article 4Transitional Measures Accompanying the Partial Amendment of the Ordinance for Enforcement of the Employment Insurance Act
第四条（雇用保険法施行規則の一部改正に伴う経過措置）
Documents that are actually submitted or issued at the time of enforcement of this Ministerial Order and are used in accordance with the forms (referred to as the "Former Forms" in the following paragraph) of the Ordinance for Enforcement of the Employment Insurance Act before amendment by the provisions of Article 4 are deemed to be in accordance with the forms of the Ordinance for Enforcement of the Employment Insurance Act as amended by the provisions of that Article.
この省令の施行の際現に提出され、又は交付されている第四条の規定による改正前の雇用保険法施行規則の様式（次項において「旧様式」という。）により使用されている書類は、同条の規定による改正後の雇用保険法施行規則の様式によるものとみなす。
Paper forms in accordance with the Former Forms that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧様式による用紙については、当分の間、これを取り繕って使用することができる。
