法第十九条第二項の規定による認定又は法第二十条第一項但書前段の場合に同条第三項の規定により準用する法第十九条第二項の規定による認定は様式第二号により、法第二十条第一項但書後段の場合に同条第三項の規定により準用する法第十九条第二項の規定による認定は様式第三号により、所轄労働基準監督署長から受けなければならない。
A certification under the provisions of paragraph (2) of Article l9 of the Act; or a certification under the provisions of paragraph (2) of Article 19 of the Act applied mutatis mutandis provided in the provisions of paragraph (3) of Article 20 of the Act in the case of the first half of the proviso of paragraph (1) of Article 20 of the Act shall be obtained according to Form No. 2, and a certification under the provisions of paragraph (2) of Article 19 of the Act applied mutatis mutandis provided in the provisions of paragraph (3) of Article 20 of the Act in the case of the second half of the proviso of paragraph (1) of Article 20 of the Act the Director of the Labor Standards Office Concerned shall be obtained according to Form No. 3, from the Director of the Labor Standards Office Concerned.
