第一条（施行期日）
Supplementary Provisions, Article 1Effective Date
この省令は、労働安全衛生法等の一部を改正する法律の施行の日（平成十八年四月一日）から施行する。
This Ministerial Ordinance comes into effect as of the date on which the Act Partially Amending the Industrial Safety and Health Act and Other Acts comes into effect (April 1, 2006).
第二条（経過措置）
Supplementary Provisions, Article 2Transitional Measures
この省令の施行の際現にあるこの省令による改正前の様式（次項において「旧様式」という。）により使用されている書類は、この省令による改正後の様式によるものとみなす。
Documents that actually exist at the time this Ministerial Ordinance comes into effect and are in use in accordance with the Forms prior to the amendment by this Ministerial Ordinance (referred to as the "former Forms" in the following paragraph) are deemed to be documents in accordance with the Forms as amended by this Ministerial Ordinance.
この省令の施行の際現にある旧様式による用紙は、当分の間、これを取り繕って使用することができる。
The paper forms in accordance with the former Forms that actually exist at the time this Ministerial Ordinance comes into effect may be used for the time being after making the necessary corrections.
