第五条（様式に関する経過措置）
Supplementary Provisions, Article 5Transitional Measures Concerning Forms
第一条の規定による改正前の労働基準法施行規則第五十二条の規定による証票、第十二条による改正前の労働保険の保険料の徴収等に関する法律施行規則第七十三条の規定による証票、第十四条の規定による改正前の労働安全衛生規則第九十五条の三の規定による証票、第二十二条の規定による改正前の産業安全専門官及び労働衛生専門官規程第五条の規定による証票並びに第二十四条による改正前の雇用保険法施行規則第十七条の七及び第百四十四条の証明書は、当分の間、それぞれ、第一条の規定による改正後の労働基準法施行規則第五十二条の規定による証票、第十二条による改正後の労働保険の保険料の徴収等に関する法律施行規則第七十三条の規定による証票、第十四条の規定による改正後の労働安全衛生規則第九十五条の三の規定による証票、第二十二条の規定による改正後の産業安全専門官及び労働衛生専門官規程第五条の規定による証票並びに第二十四条の規定による改正後の雇用保険法施行規則第十七条の七及び第百四十四条の規定による証明書とみなす。
The identification card under the provisions of Article 52 of the Ordinance for Enforcement of the Labor Standards Act prior to the amendment by the provisions of Article 1, the identification card under the provisions of Article 73 of the Ordinance for Enforcement of the Act on the Collection, etc. of Labor Insurance Premiums prior to the amendment by Article 12, the identification card under the provisions of Article 95-3 of the Ordinance on Industrial Safety and Health prior to the amendment by the provisions of Article 14, the identification card under the provisions of Article 5 of the Rules on Industrial Safety Specialists and Industrial Health Specialists prior to the amendment by the provisions of Article 22, and the certificates under Article 17-7 and Article 144 of the Ordinance for Enforcement of the Employment Insurance Act prior to the amendment by Article 24 are, for the time being, respectively deemed to be the identification card under the provisions of Article 52 of the Ordinance for Enforcement of the Labor Standards Act as amended by the provisions of Article 1, the identification card under the provisions of Article 73 of the Ordinance for Enforcement of the Act on the Collection, etc. of Labor Insurance Premiums as amended by Article 12, the identification card under the provisions of Article 95-3 of the Ordinance on Industrial Safety and Health as amended by the provisions of Article 14, the identification card under the provisions of Article 5 of the Rules on Industrial Safety Specialists and Industrial Health Specialists as amended by the provisions of Article 22, and the certificates under the provisions of Article 17-7 and Article 144 of the Ordinance for Enforcement of the Employment Insurance Act as amended by the provisions of Article 24.
