The requests set forth in the following items (hereinafter in this Subsection referred to as "requests for approval of transfer") must be made by disclosing the matters provided for in these items:
次の各号に掲げる請求（以下この款において「譲渡等承認請求」という。）は、当該各号に定める事項を明らかにしてしなければならない。

新株予約権を、それを発行した会社以外の人から取得した人のことです（発行した会社自身は除きます）。この人は会社に、新株予約権原簿への記載や記録を請求できます。
