A person seeking the registration referred to in the preceding Article must submit a written application for registration to the Prime Minister, in which it states the following particulars:
前条の登録を受けようとする者は、次に掲げる事項を記載した登録申請書を内閣総理大臣に提出しなければならない。
its trade name or name;
商号、名称又は氏名
particulars concerning its financial basis specified by Cabinet Office Order;
財産的基礎に係る事項として内閣府令で定めるもの
the name and location of its principal business office or office (or if it is a foreign corporation or an individual with a domicile in a foreign country, its principal business office or office and its principal business office or office in Japan);
主たる営業所又は事務所（外国法人又は外国に住所を有する個人にあつては、主たる営業所又は事務所及び国内における主たる営業所又は事務所）の名称及び所在地
the name and location of the business office or office at which the investment management-related services contracting business subject to the application for registration is conducted;
登録申請の対象となる投資運用関係業務受託業を行う営業所又は事務所の名称及び所在地
the type of business (meaning the type of business set forth in the items of Article 2, paragraph (43));
業務の種別（第二条第四十三項各号に掲げる業務の種別をいう。）
if the person engages in other business, the business type;
他に事業を行つているときは、その事業の種類
other particulars specified by Cabinet Office Order.
その他内閣府令で定める事項
The following documents must accompany the written application for registration referred to in the preceding paragraph:
前項の登録申請書には、次に掲げる書類を添付しなければならない。
a document pledging that the person does not fall under any of the items of Article 66-74 (excluding items (ii) through (v), item (vii), (c) and item (viii), (c));

a document in which the content and method of the investment management-related services contracting business specified by Cabinet Office Order are stated;

As concerns the documents set forth in item (iii) of the preceding paragraph accompanying a written application for registration, if the articles of incorporation have been prepared as electronic or magnetic records, such electronic or magnetic records (limited to those specified by Cabinet Office Order) may accompany the written application in lieu of paper documents.

登録の拒否
内閣総理大臣は、登録申請者が次の各号のいずれかに該当するとき、又は登録申請書若しくはこれに添付すべき書類若しくは電磁的記録のうちに虚偽の記載若しくは記録があり、若しくは重要な事実の記載若しくは記録が欠けているときは、その登録を拒否しなければならない。

定款

内閣総理大臣
