Supplementary Provisions, Article 5
An employer who intends to continue to purchase employment insurance stamps on or after August 1, 1994 must, during the period from July 1 of that year to the last day of that month, submit the written application for issuance of an employment insurance stamp purchase book prescribed in Article 42, paragraph (1) of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance to the Chief of the Public Employment Security Office having jurisdiction over the location of the workplace, together with the former purchase book, and receive the issuance of the employment insurance stamp purchase book under Article 42, paragraph (1) of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance after amendment (hereinafter referred to as the "new purchase book"). In this case, the new purchase book is to be effective on or after August 1 of that year.
平成六年八月一日以後引き続き雇用保険印紙を購入しようとする事業主は、同年七月一日から同月末日までの間に、旧通帳を添えて、労働保険の保険料の徴収等に関する法律第四十二条第一項に規定する雇用保険印紙購入通帳交付申請書を事業場の所在地を管轄する公共職業安定所長に提出して、改正後の労働保険の保険料の徴収等に関する法律施行規則第四十二条第一項の規定による雇用保険印紙購入通帳（以下「新通帳」という。）の交付を受けなければならない。この場合において、新通帳は、同年八月一日以後、その効力を有する。
