在留カードの記載事項は、次に掲げる事項とする。
The required information on the residence card is the following particulars:
氏名、生年月日、性別及び国籍の属する国又は第二条第五号ロに規定する地域
name, date of birth, sex, country of nationality or a region provided for in Article 2, item (v), sub-item (b);
住居地（本邦における主たる住居の所在地をいう。以下同じ。）
place of residence (meaning the location of the main place of residence in Japan; the same applies hereinafter);
在留資格及び在留期間の満了の日
status of residence and expiration date of the period of stay;
在留カードの番号及び有効期間の満了の日
number and expiration date of the validity period of the residence card;
就労制限の有無
whether or not there are any restrictions on work; and
第十九条第二項の規定による許可を受けているときは、その旨
if the permission pursuant to the provisions of Article 19, paragraph (2) has been granted to that effect.
その他法務省令で定める事項
other particulars provided for by Ministry of Justice Order.
前項第四号の在留カードの番号は、法務省令で定めるところにより、在留カードの交付（再交付を含む。）ごとに異なる番号を定めるものとする。
The number of the residence card set forth in item (iv) of the preceding paragraph is to be established as a different number for each residence card issued (including reissuance), pursuant to the provisions of Ministry of Justice Order.
在留カードには、交付の日において本人の年齢が法務省令で定める年齢に満たない場合を除き、法務省令で定めるところにより、中長期在留者の写真を表示するものとする。この場合において、出入国在留管理庁長官は、第六条第三項の規定その他法務省令で定める法令の規定により当該中長期在留者から提供された写真を利用することができる。
Except where the age of the person on the date of issuance is under the age provided for by Ministry of Justice Order, the residence card is to display a photograph of the mid- to long-term resident pursuant to the provisions of Ministry of Justice Order. In this case, the Commissioner of the Immigration Services Agency may use the photograph that was provided by the mid- to long-term resident pursuant to the provisions of Article 6, paragraph (3) and other provisions of the laws and regulations prescribed by Ministry of Justice Order.
The method of entering the particulars to be entered pursuant to the provisions of paragraph (1), the method of displaying the photograph to be displayed pursuant to the provisions of the preceding paragraph, the form of the residence card and other necessary particulars concerning the residence card are provided for by Ministry of Justice Order.
The Commissioner of the Immigration Services Agency is to record the following particulars on the residence card in an electronic or magnetic means pursuant to the provisions of Ministry of Justice Order, in addition to the particulars set forth in the items of paragraph (1) and the particulars pertaining to the photograph displayed pursuant to the provisions of paragraph (3):
許可の種類及び年月日
type and date of permission;
在留カードの交付年月日
date of issuance of the residence card;
前三号に掲げるもののほか、法務省令で定める事項
beyond what is set forth in the preceding three items, particulars provided for by Ministry of Justice Order; and
the fact that the Commissioner of the Immigration Services Agency has recorded the particulars set forth in the items of paragraph (1), the particulars pertaining to the photograph displayed pursuant to the provisions of paragraph (3) and the particulars set forth in the preceding items.
