前条第一項の登録を受けようとする者は、法務省令で定めるところにより、次に掲げる事項を記載した申請書を出入国在留管理庁長官に提出しなければならない。
A person who seeks to obtain the registration set forth under paragraph (1) of the preceding Article must submit a written application stating the particulars set forth in the following items to the Commissioner of the Immigration Services Agency pursuant to the provisions of Ministry of Justice Order:
氏名又は名称及び住所並びに法人にあつては、その代表者の氏名
the name and address, and in the case of a corporation, the name of its representative;
支援業務を行う事務所の所在地
the location of the business office that is to engage in the support work;
支援業務の内容及びその実施方法その他支援業務に関し法務省令で定める事項
the content of the support work and its implementation method, and other particulars specified in Ministry of Justice Order concerning support work.
前項の申請書には、前条第一項の登録を受けようとする者が第十九条の二十六第一項各号のいずれにも該当しないことを誓約する書面その他の法務省令で定める書類を添付しなければならない。
A document pledging that the person seeking to register as provided for in paragraph (1) of the preceding Article does not fall under any of the items of Article 19-26, paragraph (1) and other documents specified in Ministry of Justice Order must be attached to the written application set forth under the preceding paragraph.
