新株予約権証券には、次に掲げる事項及びその番号を記載し、株式会社の代表取締役（指名委員会等設置会社にあっては、代表執行役）がこれに署名し、又は記名押印しなければならない。
A stock company must state the following matters and the serial number on a share option certificates and the representative director of the stock company (or the representative executive officer for a company with nominating committee, etc.) must affix the representative director's signature, or name and seal:
株式会社の商号
the trade name of the stock company; and
当該新株予約権証券に係る証券発行新株予約権の内容及び数
the features and number of share options for which certificates are issued relating to relevant share option certificates.
