第二十二条（転入届に当たり特別の事項を届け出なければならない者等）
Article 22Persons Required to Notify Special Matters upon Moving-in Notification, etc.
The persons specified by Cabinet Order prescribed in Article 22, paragraph (1), item (vii) of the Act are persons who are unable to submit the document referred to in paragraph (2) of that Article because they are not recorded in the basic resident register of any municipality or for other unavoidable reasons, and the matters specified by Cabinet Order prescribed in that item are the date of birth, sex and identification of the family register.
法第二十二条第一項第七号に規定する政令で定める者はいずれの市町村の住民基本台帳にも記録されていないことその他やむを得ない理由により同条第二項の文書を提出することができない者とし、同号に規定する政令で定める事項は出生の年月日、男女の別及び戸籍の表示とする。
Article 23Certificate of Moving Out
The document concerning a change of domicile specified by Cabinet Order prescribed in Article 22, paragraph (2) of the Act is the certification of moving out prepared by the mayor of the municipality of the previous address (hereinafter referred to as the "certificate of moving out").
法第二十二条第二項に規定する住所の異動に関する文書で政令で定めるものは、前住所地の市町村長が作成する転出の証明書（以下「転出証明書」という。）とする。
In addition to the matters set out in Article 7, items (i) through (v), item (viii)-2 and item (xiii) of the Act, the following matters must be stated in a certificate of moving out.
転出証明書には、法第七条第一号から第五号まで、第八号の二及び第十三号に掲げる事項のほか、次に掲げる事項を記載しなければならない。

住所
The destination of the move and the scheduled date of moving out;
転出先及び転出の予定年月日
For a person who is an insured person under National Health Insurance, a statement to that effect;
国民健康保険の被保険者である者については、その旨
For a person who is an insured person under latter-stage elderly medical care, a statement to that effect;
後期高齢者医療の被保険者である者については、その旨
For a person who is an insured person under Long-Term Care Insurance, a statement to that effect;
介護保険の被保険者である者については、その旨
For a person who is an insured person under National Pension, the category of insured person under National Pension and the Basic Pension Number;
国民年金の被保険者である者については、国民年金の被保険者の種別及び基礎年金番号
For a person who receives Child Allowance, a statement to that effect.
児童手当の支給を受けている者については、その旨
Article 24Issuance, etc. of Certificate of Moving Out
When a moving-out notification has been made (excluding when the main clause of Article 24-2, paragraph (1) of the Act or the main clause of paragraph (2) of that Article applies, or when the person moves out of Japan), the mayor of the municipality must issue a certificate of moving out.
市町村長は、転出届があつたとき（法第二十四条の二第一項本文若しくは同条第二項本文の規定の適用を受けるとき又は国外に転出をするときを除く。）は、転出証明書を交付しなければならない。
A person who has been issued a certificate of moving out may receive reissuance of it if the person has lost, destroyed, soiled or damaged it.
転出証明書の交付を受けた者は、転出証明書を亡失し、滅失し、汚損し、又は破損したときは、その再交付を受けることができる。
第二十四条の二（最初の転入届等において特例の適用を受けることができない場合）
Article 24-2Cases Where the Special Provisions Cannot Apply to the First Moving-in Notification, etc.
The cases specified by Cabinet Order as prescribed in the proviso to Article 24-2, paragraph (1) of the Act are the following cases:
法第二十四条の二第一項ただし書に規定する政令で定める場合は、次に掲げる場合とする。
A case where a person who has made a moving-out notification has been recorded in the basic resident register of any municipality during the period from the making of that moving-out notification until the first moving-in notification (meaning the first moving-in notification prescribed in Article 24-2, paragraph (1) of the Act; the same applies hereinafter) is made;
転出届をした者が、当該転出届がされてから最初の転入届（法第二十四条の二第一項に規定する最初の転入届をいう。以下同じ。）がされるまでの間において、いずれかの市町村の住民基本台帳に記録されたことがある場合
A case where a person who has made a moving-out notification makes the first moving-in notification on or after the earlier of the day on which 30 days have elapsed from the scheduled date of moving out notified by that moving-out notification and the day on which 14 days have elapsed from the day of moving in;
A case where, at the time of the first moving-in notification, the Individual Number Card (meaning the Individual Number Card prescribed in Article 2, paragraph (7) of the Number Use Act; the same applies hereinafter) has not been submitted under Article 17, paragraph (6) of the Number Use Act.
最初の転入届の際に、番号利用法第十七条第六項の規定による個人番号カード（番号利用法第二条第七項に規定する個人番号カードをいう。以下同じ。）の提出がされなかつた場合
The cases specified by Cabinet Order as prescribed in the proviso to Article 24-2, paragraph (2) of the Act are the following cases:

転出届をした世帯員（法第二十四条の二第二項に規定する世帯員をいう。以下この項において同じ。）が、当該転出届がされてから最初の世帯員に関する転入届（同条第二項に規定する最初の世帯員に関する転入届をいう。以下同じ。）がされるまでの間において、いずれかの市町村の住民基本台帳に記録されたことがある場合
A case where the householder of the household to which a household member who has made a moving-out notification belongs makes the first moving-in notification concerning a household member on or after the earlier of the day on which 30 days have elapsed from the scheduled date of moving out notified by that moving-out notification and the day on which 14 days have elapsed from the day of moving in;
転出届をした世帯員が属する世帯の世帯主が、当該転出届により届け出た転出の予定年月日から三十日を経過した日又は転入をした日から十四日を経過した日のいずれか早い日以後に、最初の世帯員に関する転入届をする場合
A case where, at the time of the first moving-in notification concerning a household member, the Individual Number Card has not been submitted under Article 17, paragraph (6) of the Number Use Act for the householder of the household to which the household member who has made a moving-out notification belongs.
最初の世帯員に関する転入届の際に、転出届をした世帯員が属する世帯の世帯主について番号利用法第十七条第六項の規定による個人番号カードの提出がされなかつた場合
第二十四条の三（転出地市町村長から転入予定地市町村長への通知事項）

The matters specified by Cabinet Order as prescribed in Article 24-2, paragraph (3) of the Act are, in addition to the matters set out in Article 7, items (i) through (v), item (viii)-2 and item (xiii) of the Act, the following matters:
法第二十四条の二第三項に規定する政令で定める事項は、法第七条第一号から第五号まで、第八号の二及び第十三号に掲げる事項のほか、次に掲げる事項とする。

For a person who has been issued an Individual Number Card, the date of issuance of that Individual Number Card, the date on which its period of validity expires, and other matters specified by Order of the Ministry of Internal Affairs and Communications as necessary for the management of Individual Number Cards.
個人番号カードの交付を受けている者については、当該個人番号カードの発行の日及び有効期間が満了する日その他個人番号カードの管理のために必要な事項として総務省令で定めるもの
第二十四条の四（転出地市町村長から転入予定地市町村長への通知事項の保存期間）
Article 24-4Retention Period of Matters Notified by the Mayor of the Municipality of Departure to the Mayor of the Intended Destination Municipality
The period specified by Cabinet Order as prescribed in Article 24-2, paragraph (4) of the Act is the period from the day on which the notification under paragraph (3) of that Article was given until the day on which 30 days have elapsed from the scheduled date of moving out notified under that paragraph.
法第二十四条の二第四項に規定する政令で定める期間は、同条第三項の規定による通知があつた日から、同項の規定により通知された転出の予定年月日から三十日を経過した日までの期間とする。
Article 25Persons Not Required to Submit a Notification of Household Change
The persons specified by Cabinet Order as prescribed in Article 25 of the Act are persons who, as a result of the number of persons belonging to the household other than the householder having become one, are that sole person.
法第二十五条に規定する政令で定める者は、世帯主以外のその世帯に属する者が一人になつた場合におけるその者とする。
Article 26Method of Notification
A notification under Chapter IV or Chapter IV-4 of the Act must be made in a document stating the address of the person actually handling the notification and the date of the notification, and signed by the person handling the notification.
法第四章又は第四章の四の規定による届出は、現に届出の任に当たつている者の住所及び届出の年月日が記載され、並びに当該届出の任に当たつている者が署名した書面でしなければならない。

国民年金法第七条第一項第一号に規定する第一号被保険者又は前条に規定する法令の規定による国民年金の被保険者のいずれであるかの区別

同条第二項に規定する最初の世帯員に関する転入届
わかりやすく言うと
個人番号カードを持つ世帯主の転出届に併せて、カードを持たない世帯員が転出届をした場合に、その後世帯員が最初に行う転入届で、世帯主が自分の最初の転入届に併せて世帯員に代わって行うもののことです（住民基本台帳法第二十四条の二第二項）。
戸籍
人の出生や婚姻、親子関係などを記録する公の帳簿で、本籍を定めた夫婦と、その夫婦と同じ氏の子ごとに作られます。外国人と結婚した人や配偶者のいない人について新しく戸籍を作るときは、その人と、その人と同じ氏の子ごとに作られます（戸籍法第六条）。戸籍の事務は、法律に別の定めがある場合を除き、市町村長（東京都の特別区や政令指定都市の区では区長）が行います（同法第一条・第四条）。一人ひとりについて、本籍のほか、氏名とその振り仮名、生年月日、戸籍に入った理由と日付、父母、養子であれば養親、夫婦であることなどが記載されます（同法第十三条）。記載は、出生・婚姻・離婚・死亡などの届出（同法第四十九条・第七十四条・第七十六条・第八十六条）や、報告、申請、裁判などにもとづいて行われます（同法第十五条）。日本人と外国人の婚姻の届出があったときは、夫婦ではなくその日本人について新しい戸籍が作られます。ただし、その日本人がすでに自分の戸籍の筆頭に記載されているときは、新しい戸籍は作られません（同法第六条・第十六条第三項）。日本に住所のある中長期在留者や特別永住者などの外国人は、その市町村の住民票に記載されます。その住民票には、戸籍の表示の代わりに国籍などが記載され、中長期在留者であれば在留資格や在留期間も記載されます（住民基本台帳法第三十条の四十五）。これらにあたらない外国人は、住民票に記載されません（同法第三十九条）。
