The document concerning a change of domicile specified by Cabinet Order prescribed in Article 22, paragraph (2) of the Act is the certification of moving out prepared by the mayor of the municipality of the previous address (hereinafter referred to as the "certificate of moving out").
法第二十二条第二項に規定する住所の異動に関する文書で政令で定めるものは、前住所地の市町村長が作成する転出の証明書（以下「転出証明書」という。）とする。
In addition to the matters set out in Article 7, items (i) through (v), item (viii)-2 and item (xiii) of the Act, the following matters must be stated in a certificate of moving out.
転出証明書には、法第七条第一号から第五号まで、第八号の二及び第十三号に掲げる事項のほか、次に掲げる事項を記載しなければならない。

For a person who is an insured person under National Health Insurance, a statement to that effect;
国民健康保険の被保険者である者については、その旨
For a person who is an insured person under latter-stage elderly medical care, a statement to that effect;
後期高齢者医療の被保険者である者については、その旨
For a person who is an insured person under Long-Term Care Insurance, a statement to that effect;
介護保険の被保険者である者については、その旨
For a person who is an insured person under National Pension, the category of insured person under National Pension and the Basic Pension Number;
国民年金の被保険者である者については、国民年金の被保険者の種別及び基礎年金番号
For a person who receives Child Allowance, a statement to that effect.
児童手当の支給を受けている者については、その旨
