第二十八条（国民年金の被保険者である者に係る届出の付記事項）
Article 28Matters to Be Additionally Stated in a Notification Concerning a Person Who Is an Insured Person under National Pension
The matters specified by Cabinet Order as prescribed in Article 29 of the Act are the matters listed in each of the following items, in accordance with the classification of notification listed in that item.
法第二十九条に規定する政令で定める事項は、次の各号に掲げる届出の区分に応じ、当該各号に掲げる事項とする。
Moving-in notification and notification under Article 30-46 of the Act: the following matters;
For a person who has continued to be an insured person under National Pension of the same category since the previous address, the category of insured person under National Pension and, if the person is a person prescribed in Article 22, paragraph (1), item (vii) of the Act or a person making the notification under Article 30-46, the Basic Pension Number;
前住所地から引き続き同一の種別の国民年金の被保険者である者にあつては、当該国民年金の被保険者の種別及びその者が法第二十二条第一項第七号に規定する者又は第三十条の四十六の規定による届出を行う者である場合には、基礎年金番号
For a person whose category of insured person under National Pension has changed by reason of moving in, the category of insured person under National Pension after the change and, if the person is a person prescribed in Article 22, paragraph (1), item (vii) of the Act or a person making the notification under Article 30-46, the Basic Pension Number;
転入により国民年金の被保険者の種別に変更があつた者にあつては、変更後の国民年金の被保険者の種別及びその者が法第二十二条第一項第七号に規定する者又は第三十条の四十六の規定による届出を行う者である場合には、基礎年金番号
For a person who has become an insured person under National Pension by moving in, the category of insured person under National Pension and, if the person has previously been an insured person under National Pension, the Basic Pension Number and, if the person's name changed while the person was not an insured person under National Pension, the name at the time the person last ceased to be an insured person under National Pension;
転入により国民年金の被保険者となつた者にあつては、国民年金の被保険者の種別並びにその者が前に国民年金の被保険者であつたことがある者である場合には、基礎年金番号及び国民年金の被保険者でなかつた間に氏名の変更があつたときは、最後に国民年金の被保険者でなくなつた当時の氏名
Notification of a move within a municipality and moving-out notification: an indication that the person is an insured person under National Pension;
Notification under Article 30-47 of the Act: the following matters.
法第三十条の四十七の規定による届出 次に掲げる事項
For a person who has continued to be an insured person under National Pension of the same category since before becoming a mid- to long-term resident, etc., the category of insured person under National Pension and the Basic Pension Number;
中長期在留者等となる前から引き続き同一の種別の国民年金の被保険者である者にあつては、当該国民年金の被保険者の種別及び基礎年金番号
For a person whose category of insured person under National Pension has changed by reason of becoming a mid- to long-term resident, etc., the category of insured person under National Pension after the change and the Basic Pension Number;
中長期在留者等となつたことにより国民年金の被保険者の種別に変更があつた者にあつては、変更後の国民年金の被保険者の種別及び基礎年金番号
For a person who has become an insured person under National Pension by reason of becoming a mid- to long-term resident, etc., the category of insured person under National Pension and, if the person has previously been an insured person under National Pension, the Basic Pension Number and, if the person's name changed while the person was not an insured person under National Pension, the name at the time the person last ceased to be an insured person under National Pension.
中長期在留者等となつたことにより国民年金の被保険者となつた者にあつては、国民年金の被保険者の種別並びにその者が前に国民年金の被保険者であつたことがある者である場合には、基礎年金番号及び国民年金の被保険者でなかつた間に氏名の変更があつたときは、最後に国民年金の被保険者でなくなつた当時の氏名

国民年金法第七条第一項第一号に規定する第一号被保険者又は前条に規定する法令の規定による国民年金の被保険者のいずれであるかの区別
わかりやすく言うと
国民年金の被保険者が、第一号被保険者（国内に住む二十歳以上六十歳未満の人のうち、厚生年金の被保険者やその被扶養配偶者などでない人）か、国民年金法附則第五条により申し出て被保険者となった人かの区別のことです。

国民年金法第十四条に規定する基礎年金番号
わかりやすく言うと
政府が管掌する国民年金事業と厚生年金保険事業の運営などの事務に用いる記号と番号で、厚生労働省令で定めるもののことです（国民年金法第十四条）。

転出をする人が、あらかじめ、自分の氏名、転出先及び転出の予定年月日を市町村長に届け出ることです（住民基本台帳法第二十四条）。

わかりやすく言うと
新たにある市町村の区域内に住所を定めた（転入した）人が、転入をした日から十四日以内に、氏名、住所、従前の住所などを市町村長に届け出ることです。出生による場合は転入に含まれません（住民基本台帳法第二十二条）。

法第三十条の四十六に規定する中長期在留者等
わかりやすく言うと
日本の国籍を持たない人のうち、中長期在留者、特別永住者、一時庇護許可者と仮滞在許可者のことで、出生や国籍喪失による経過滞在者は含みません（住民基本台帳法第三十条の四十六）。
