第二十七条（国民健康保険の被保険者である者に係る付記事項）
Article 27Matters to Be Additionally Stated Concerning a Person Who Is an Insured Person under National Health Insurance
The matters specified by Cabinet Order as prescribed in Article 28 of the Act are the matters prescribed in each of the following items, in accordance with the classification of notification listed in that item.
法第二十八条に規定する政令で定める事項は、次の各号に掲げる届出の区分に応じ、当該各号に定める事項とする。
Notification under Article 22 of the Act (hereinafter referred to as "moving-in notification") (excluding the notification listed in item (iii)), notification under Article 30-46 of the Act, and notification under Article 30-47 of the Act (excluding the notification listed in item (iv)): the following matters;
An indication that the person has acquired qualification as an insured person under National Health Insurance;
国民健康保険の被保険者の資格を取得した旨
Occupation;
職業
If there is already a person who has acquired qualification as an insured person under National Health Insurance in the household to which the person has come to belong, the insured person's symbol and number (meaning the insured person's symbol and number prescribed in Article 111-2, paragraph (1) of the National Health Insurance Act; the same applies hereinafter in this Article) pertaining to any one of the insured persons belonging to that household;
その者が属することとなつた世帯に既に国民健康保険の被保険者の資格を取得している者がある場合には、その世帯に属する被保険者のうちいずれかの者に係る被保険者記号・番号（国民健康保険法第百十一条の二第一項に規定する被保険者記号・番号をいう。以下この条において同じ。）
In the case prescribed in (c), if the householder of that household is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;
ハに規定する場合において、当該世帯の世帯主が国民健康保険法第五十四条の三第一項又は第二項本文の規定の適用を受けているときは、その旨
Notification under Article 23 of the Act (hereinafter referred to as "notification of a move within a municipality" in this Chapter and Article 30-19), moving-out notification, and notification under Article 25 of the Act (referred to as "notification of household change" in item (ii) of the following Article and item (ii) of Article 27-3): the following matters;
The insured person's symbol and number pertaining to the person;
その者に係る被保険者記号・番号
If the householder of the household to which the person belongs is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;
その者が属する世帯の世帯主が国民健康保険法第五十四条の三第一項又は第二項本文の規定の適用を受けている場合には、その旨
Moving-in notification (limited to one pertaining to changing an address within the area of a prefecture): the following matters;
転入届（一の都道府県の区域内において住所を変更することに係るものに限る。） 次に掲げる事項
The date on which the person acquired qualification as an insured person under National Health Insurance;
国民健康保険の被保険者の資格を取得した年月日
If there is already a person who has acquired qualification as an insured person under National Health Insurance in the household to which the person has come to belong, the insured person's symbol and number pertaining to any one of the insured persons belonging to that household;
その者が属することとなつた世帯に既に国民健康保険の被保険者の資格を取得している者がある場合には、その世帯に属する被保険者のうちいずれかの者に係る被保険者記号・番号
In the case prescribed in (b), if the householder of that household is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;
ロに規定する場合において、当該世帯の世帯主が国民健康保険法第五十四条の三第一項又は第二項本文の規定の適用を受けているときは、その旨
Notification under Article 30-47 of the Act (limited to where the person making that notification has continuously held qualification as an insured person under National Health Insurance since before becoming a mid- to long-term resident, etc. (meaning a mid- to long-term resident, etc. prescribed in Article 30-46 of the Act; the same applies from the following Article through Article 28)): the following matters;
法第三十条の四十七の規定による届出（当該届出をする者が中長期在留者等（法第三十条の四十六に規定する中長期在留者等をいう。次条から第二十八条までにおいて同じ。）となる前から引き続き国民健康保険の被保険者の資格を有する場合に限る。） 次に掲げる事項

わかりやすく言うと
新たにある市町村の区域内に住所を定めた（転入した）人が、転入をした日から十四日以内に、氏名、住所、従前の住所などを市町村長に届け出ることです。出生による場合は転入に含まれません（住民基本台帳法第二十二条）。
