Supplementary Provisions, Article 7
With regard to the application of Article 30-14, paragraphs (1) and (2) of the Order for the Enforcement of the Residential Basic Book Act to a person, among the persons whose surname has changed prescribed in Article 30-14, paragraph (1) of that Order (including where it is applied by replacing terms under the preceding Article; the same applies hereinafter in this paragraph) (referred to as "person whose surname has changed" in paragraph (1) of the following Article), for whom no kana reading of the former surname is entered or recorded in the family register or removed family register in which the former surname the person wishes to request be entered in the certificate of residence is entered or recorded, the phrases listed in the middle column of the following table in the provisions listed in the left-hand column of the same table are deemed to be replaced with the phrases listed in the right-hand column of the same table, respectively.

The mayor of the municipality of domicile who has received a request under Article 30-14, paragraph (1) of the Order for the Enforcement of the Residential Basic Book Act, applied by replacing terms under the preceding paragraph, is to enter in the certificate of residence the characters indicating the reading of the characters used in the former surname pertaining to the request, as the kana reading of the former surname; in this case, for the application of Articles 30-13 and 30-14 of that Order and other provisions of laws and regulations to the person for whom that entry has been made, the characters so entered are deemed to be the kana reading of the former surname.
前項の規定により読み替えて適用する住民基本台帳法施行令第三十条の十四第一項の請求を受けた住所地市町村長は、当該請求に係る旧氏に用いられる文字の読み方を示す文字を、旧氏の振り仮名として住民票に記載をするものとする。この場合において、当該記載がされた者に係る同令第三十条の十三及び第三十条の十四の規定その他の法令の規定の適用については、当該記載がされた文字を旧氏の振り仮名とみなす。

その人が記録されている住民基本台帳を備える市町村の市町村長、つまりその人が住民登録をしている市町村の市町村長のことです（住民基本台帳法施行令第三十条の十四第一項）。

わかりやすく言うと
氏が変わったことがある人のうち、住民票に旧氏と旧氏の振り仮名の記載を求めることができる人のことで、住民票に旧氏と旧氏の振り仮名がすでに記載されている人などは除かれます（住民基本台帳法施行令第三十条の十四第一項）。
戸籍
人の出生や婚姻、親子関係などを記録する公の帳簿で、本籍を定めた夫婦と、その夫婦と同じ氏の子ごとに作られます。外国人と結婚した人や配偶者のいない人について新しく戸籍を作るときは、その人と、その人と同じ氏の子ごとに作られます（戸籍法第六条）。戸籍の事務は、法律に別の定めがある場合を除き、市町村長（東京都の特別区や政令指定都市の区では区長）が行います（同法第一条・第四条）。一人ひとりについて、本籍のほか、氏名とその振り仮名、生年月日、戸籍に入った理由と日付、父母、養子であれば養親、夫婦であることなどが記載されます（同法第十三条）。記載は、出生・婚姻・離婚・死亡などの届出（同法第四十九条・第七十四条・第七十六条・第八十六条）や、報告、申請、裁判などにもとづいて行われます（同法第十五条）。日本人と外国人の婚姻の届出があったときは、夫婦ではなくその日本人について新しい戸籍が作られます。ただし、その日本人がすでに自分の戸籍の筆頭に記載されているときは、新しい戸籍は作られません（同法第六条・第十六条第三項）。日本に住所のある中長期在留者や特別永住者などの外国人は、その市町村の住民票に記載されます。その住民票には、戸籍の表示の代わりに国籍などが記載され、中長期在留者であれば在留資格や在留期間も記載されます（住民基本台帳法第三十条の四十五）。これらにあたらない外国人は、住民票に記載されません（同法第三十九条）。
