第三十条の五（都道府県知事に通知する住民票の記載等に関する事項）
Article 30-5Matters Concerning Entry in the Certificate of Residence, etc. to Be Notified to the Prefectural Governor
The matters concerning entry in the certificate of residence, etc. prescribed in Article 30-6, paragraph (1) of the Act that are specified by Cabinet Order are the matters prescribed in each of the following items, in accordance with the classification of cases listed in that item.
法第三十条の六第一項に規定する住民票の記載等に関する事項で政令で定めるものは、次の各号に掲げる場合の区分に応じ、当該各号に定める事項とする。
Where an entry in the certificate of residence has been made: an indication that an entry in the certificate of residence has been made, and the reason for the entry specified by Order of the Ministry of Internal Affairs and Communications, such as moving in, and the date on which that reason arose;
住民票の記載を行つた場合 住民票の記載を行つた旨並びに転入その他の総務省令で定める記載の事由及びその事由が生じた年月日
Where a certificate of residence has been deleted: an indication that the certificate of residence has been deleted, and the reason for the deletion specified by Order of the Ministry of Internal Affairs and Communications, such as moving out, and the date on which that reason arose (where the certificate of residence has been deleted on the basis of a moving-out notification, the scheduled date of moving out);
住民票の消除を行つた場合 住民票の消除を行つた旨並びに転出その他の総務省令で定める消除の事由及びその事由が生じた年月日（転出届に基づき住民票の消除を行つた場合にあつては、転出の予定年月日）
Where an entry has been amended with respect to all or part of the matters listed in Article 7, items (i) through (iii) and (vii) of the Act (with respect to the matters listed in that item (vii), the address): an indication that the entry in the certificate of residence has been amended, and the reason for the amendment of the entry specified by Order of the Ministry of Internal Affairs and Communications, such as a move within a municipality, and the date on which that reason arose;
法第七条第一号から第三号まで及び第七号に掲げる事項（同号に掲げる事項については、住所とする。）の全部又は一部についての記載の修正を行つた場合 住民票の記載の修正を行つた旨並びに転居その他の総務省令で定める記載の修正の事由及びその事由が生じた年月日
Where an entry has been amended with respect to the matters listed in Article 7, item (viii)-2 of the Act: an indication that the entry in the certificate of residence has been amended, the reason for the amendment of the entry specified by Order of the Ministry of Internal Affairs and Communications, such as a request for change of Individual Number, and the date on which that reason arose, and the Individual Number that was entered before the amendment of the entry in the certificate of residence (excluding the case where no Individual Number was entered in that certificate of residence);
法第七条第八号の二に掲げる事項についての記載の修正を行つた場合 住民票の記載の修正を行つた旨、個人番号の変更請求その他の総務省令で定める記載の修正の事由及びその事由が生じた年月日並びに当該住民票の記載の修正前に記載されていた個人番号（当該住民票に個人番号が記載されていなかつた場合を除く。）
Where an entry has been amended with respect to the matters listed in Article 7, item (xiii) of the Act: an indication that the entry in the certificate of residence has been amended, the reason for the amendment of the entry specified by Order of the Ministry of Internal Affairs and Communications, and the date on which that reason arose, and the resident record code that was entered before the amendment of the entry in the certificate of residence (excluding the case where no resident record code was entered in that certificate of residence).
法第七条第十三号に掲げる事項についての記載の修正を行つた場合 住民票の記載の修正を行つた旨、総務省令で定める記載の修正の事由及びその事由が生じた年月日並びに当該住民票の記載の修正前に記載されていた住民票コード（当該住民票に住民票コードが記載されていなかつた場合を除く。）

転出をする人が、あらかじめ、自分の氏名、転出先及び転出の予定年月日を市町村長に届け出ることです（住民基本台帳法第二十四条）。
