Article 30-17Entry in the Certificate of Residence of Matters Concerning the Entry and Deletion of a Foreign Resident's Commonly Used Name
In the cases listed in the following items, the mayor of the municipality of domicile must enter the matters specified in each of those items (referred to as "matters concerning the entry and deletion of a commonly used name" in the following paragraph and paragraph (3)) in the certificate of residence for the foreign resident:
In the case where a commonly used name has been entered in the certificate of residence for a foreign resident (excluding the case under paragraph (3) of the preceding Article): the name of the municipality that entered the commonly used name (in the case of a special ward, the name of the ward; the same applies in the following item) and the date;
In the case where a commonly used name entered in the certificate of residence for a foreign resident has been deleted: that commonly used name, and the name of the municipality that deleted it and the date.
In the cases listed in the following items, when making an entry in the certificate of residence for a foreign resident, the mayor of a municipality must enter the matters concerning the entry and deletion of a commonly used name specified in each of those items in the certificate of residence for the foreign resident:
市町村長は、次の各号に掲げる場合において、外国人住民に係る住民票の記載をするときは、当該各号に定める通称の記載及び削除に関する事項を当該外国人住民に係る住民票に記載をしなければならない。
In the case where a foreign resident has submitted a moving-in notification accompanied by a certificate of moving out in which the matters concerning the entry and deletion of the commonly used name of that foreign resident are entered: those matters concerning the entry and deletion of a commonly used name;

In the case where a foreign resident has submitted a first moving-in notification or a first moving-in notification concerning a household member, and the matters concerning the entry and deletion of the commonly used name of that foreign resident have been notified pursuant to Article 24-2, paragraph (3) or paragraph (6) of the Act: those matters concerning the entry and deletion of a commonly used name.
外国人住民が最初の転入届又は最初の世帯員に関する転入届をした場合において、法第二十四条の二第三項又は第六項の規定により当該外国人住民の通称の記載及び削除に関する事項が通知されたとき 当該通称の記載及び削除に関する事項

外国人住民に係る住民票に通称の記載及び削除に関する事項の記載がされている場合における第三十条の二十一の規定により読み替えて適用される第二十三条第二項及び第二十四条の三の規定の適用については、第三十条の二十一の規定により読み替えて適用される第二十三条第二項中「並びに同条の表の下欄に掲げる事項」とあるのは「、同条の表の下欄に掲げる事項並びに通称の記載及び削除に関する事項（第三十条の十七第一項に規定する通称の記載及び削除に関する事項をいう。第二十四条の三において同じ。）」と、第三十条の二十一の規定により読み替えて適用される第二十四条の三中「並びに同条の表の下欄に掲げる事項」とあるのは「、同条の表の下欄に掲げる事項並びに通称の記載及び削除に関する事項」とする。

法第二十二条第二項に規定する住所の異動に関する文書で政令で定めるものは、前住所地の市町村長が作成する転出の証明書（以下「転出証明書」という。）とする。

わかりやすく言うと
個人番号カードの交付を受けている人が転出届をした後、その人が最初に行う転入の届出（住民基本台帳法第二十二条第一項の届出）のことです（同じ法律の第二十四条の二第一項）。

同条第二項に規定する最初の世帯員に関する転入届
わかりやすく言うと
個人番号カードを持つ世帯主の転出届に併せて、カードを持たない世帯員が転出届をした場合に、その後世帯員が最初に行う転入届で、世帯主が自分の最初の転入届に併せて世帯員に代わって行うもののことです（住民基本台帳法第二十四条の二第二項）。

わかりやすく言うと

第三十条の十七第一項に規定する通称の記載及び削除に関する事項
わかりやすく言うと
外国人住民の住民票に通称を記載したときはその市町村名と年月日（転入の際に引き継いで記載した場合を除きます）、通称を削除したときはその通称と削除した市町村名及び年月日のことです（第三十条の十七第一項）。
特別区
都の区のことです（地方自治法第二百八十一条第一項）。今ある特別区は、東京都の二十三区だけです。特別区は、市町村と同じく住民に身近な地方公共団体（基礎的な地方公共団体）で、都が特別区のある区域全体でまとめて行う仕事を除き、ふつうは市町村が行う仕事を行います（同法第二百八十一条の二第二項）。市の仕事について定めた法令の規定は、その仕事を特別区が行う場合には特別区にも当てはまります（同法第二百八十一条第二項・第二百八十三条第二項）。そのまま当てはめにくいときは、政令で特別の定めを置くことができます（同法第二百八十三条第三項）。
