第十五条の三（法第十二条の四第二項及び第三項に規定する住民票の写しの交付の際の通知事項）
Article 15-3Matters to Be Notified When a Copy of a Certificate of Residence Is Issued as Prescribed in Article 12-4, paragraphs (2) and (3) of the Act
The matters specified by Cabinet Order as prescribed in Article 12-4, paragraph (2) of the Act are the following matters:
法第十二条の四第二項に規定する政令で定める事項は、次に掲げる事項とする。
The fact that a request under Article 12-4, paragraph (1) of the Act has been made;
法第十二条の四第一項の請求があつた旨
The name of the person who made the request under Article 12-4, paragraph (1) of the Act (referred to as the "requester" in the following item) and the resident record code stated in the certificate of residence relating to that person;
法第十二条の四第一項の請求をした者（次号において「請求者」という。）の氏名及びその者に係る住民票に記載された住民票コード
Among the requester and the persons belonging to the same household as the requester, the persons to be stated in the copy of the certificate of residence relating to the request under Article 12-4, paragraph (1) of the Act; and
請求者及び請求者と同一の世帯に属する者のうち、法第十二条の四第一項の請求に係る住民票の写しに記載する者
Whether there has been a request for the entry of the matters listed in Article 7, item (iv), item (viii)-2, or item (xiii) of the Act.
The matters specified by Cabinet Order as prescribed in Article 12-4, paragraph (3) of the Act are the matters listed in Article 7, items (i) through (iii) and items (vi) through (viii) of the Act that are stated in the certificate of residence (including, where a request for the entry of the matters listed in item (iv), item (viii)-2, or item (xiii) of that Article has been made, the matters for which that request was made).
法第十二条の四第三項に規定する政令で定める事項は、住民票に記載されている法第七条第一号から第三号まで及び第六号から第八号までに掲げる事項（同条第四号、第八号の二又は第十三号に掲げる事項の記載の請求があつた場合にあつては、当該請求があつた事項を含む。）とする。

わかりやすく言うと
自分が記録されている住民基本台帳のある市町村以外の市町村長に対して、自分又は同じ世帯の人の住民票の写し（一部の事項の記載を省略したもの）の交付を請求した人のことです（住民基本台帳法第十二条の四第一項）。
