The matters specified by Order of the Ministry of Health, Labour and Welfare referred to in Article 32-3, paragraph (1) of the Act (including as applied by replacing terms pursuant to the provisions of paragraphs (2) and (3) of that Article; the same applies hereinafter in this Article), item (iv) are to be the following.

If a time period when workers have to work is fixed, the starting and ending times must be specified.

If the settlement period referred to in Article 32-3, paragraph (1), item (ii) of the Act exceeds one month, provisions on the validity period of the agreement under that paragraph (excluding the case where it is made by a collective agreement, and including a resolution by a labor-management committee and a resolution by a committee on the improvement of working time arrangements)
法第三十二条の三第一項第二号の清算期間が一箇月を超えるものである場合にあつては、同項の協定（労働協約による場合を除き、労使委員会の決議及び労働時間等設定改善委員会の決議を含む。）の有効期間の定め
The notification under the provisions of Article 32-2, paragraph (2) of the Act as applied mutatis mutandis pursuant to Article 32-3, paragraph (4) of the Act must be made to the Director of the Labor Standards Office Concerned using Form No. 3-3.
法第三十二条の三第四項において準用する法第三十二条の二第二項の規定による届出は、様式第三号の三により、所轄労働基準監督署長にしなければならない。
