第一条（施行期日）
This Ministerial Ordinance comes into effect as of January 1, 2011.
この省令は、平成二十三年一月一日から施行する。
Supplementary Provisions, Article 2Transitional Measures
第二条（経過措置）
The paper forms in accordance with the Forms prior to the amendment by this Ministerial Ordinance that actually exist at the time this Ministerial Ordinance comes into effect may be used for the time being.
この省令の施行の際、現に存するこの省令による改正前の様式による用紙は、当分の間、これを使用することができる。
