Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of April 1, 2026.
この省令は、令和八年四月一日から施行する。
Supplementary Provisions, Article 2Transitional Measures
第二条（経過措置）
An application that has actually been filed at the time this Ministerial Order comes into effect using a form prior to the amendment by this Ministerial Order (referred to as "the former forms" in the following Article) is deemed to be an application filed using the form as amended by this Ministerial Order.
この省令の施行の際現に行われているこの省令による改正前の様式（次条において「旧様式」という。）による申請は、この省令による改正後の様式による申請とみなす。
Supplementary Provisions, Article 3
第三条
Paper forms in accordance with the former forms that actually exist at the time this Ministerial Order comes into effect may be used for the time being with corrections made to them.
この省令の施行の際現にある旧様式による用紙については、当分の間、これを取り繕って使用することができる。
