The notification under the provisions of paragraph (1) of Article 41-2 of the Act must be made to the Director of the Labor Standards Office Concerned according to Form No. 14-2.
法第四十一条の二第一項の規定による届出は、様式第十四号の二により、所轄労働基準監督署長にしなければならない。
The method specified by Order of the Ministry of Health, Labour and Welfare provided in the part of paragraph (1) of Article 41-2 of the Act other than the items is to be the method of obtaining the signature of the covered worker (meaning a "covered worker" provided in that paragraph; the same applies hereinafter) on a document which makes clear the following matters and receiving delivery of that document (or, if the covered worker so wishes, the method of receiving the provision of an electronic or magnetic record in which the matters to be stated in that document are recorded).

To provide face-to-face guidance by a physician (meaning ascertaining the physical and mental condition through medical interviews and other methods and giving necessary guidance through face-to-face interviews in response thereto, and excluding the face-to-face guidance under the provisions of paragraph (1) of Article 66-8-4 of the Industrial Safety and Health Act (Act No. 57 of 1972)) to covered workers whose working hours subject to health management exceed a certain number of hours.
To grant compensatory days off or special leave according to the working status and the health condition of the covered worker.
対象労働者の勤務状況及びその健康状態に応じて、代償休日又は特別な休暇を付与すること。
To establish a consultation service for covered workers concerning problems of mental and physical health.
対象労働者の心とからだの健康問題についての相談窓口を設置すること。
To give consideration to the working status and the health condition of the covered worker and, if necessary, transfer the covered worker to an appropriate department.
対象労働者の勤務状況及びその健康状態に配慮し、必要な場合には適切な部署に配置転換をすること。
To receive advice or guidance from an industrial physician or the like, or to have the covered worker receive health guidance from an industrial physician or the like.
産業医等による助言若しくは指導を受け、又は対象労働者に産業医等による保健指導を受けさせること。
The matters specified by Order of the Ministry of Health, Labour and Welfare referred to in item (x) of paragraph (1) of Article 41-2 of the Act are to be the following.
法第四十一条の二第一項第十号の厚生労働省令で定める事項は、次に掲げるものとする。
