A person seeking the designation under the provisions of paragraph (1) of the preceding Article must submit an application to the Minister of Economy, Trade and Industry, stating the following matters:
前条第一項の規定による指定を受けようとする者は、次の事項を記載した申請書を経済産業大臣に提出しなければならない。

the name and location of its principal business office or office or of the business office or office where it conducts specified credit information services;
主たる営業所又は事務所その他特定信用情報提供等業務を行う営業所又は事務所の名称及び所在地
if it does business other than in specified credit information services and business incidental to it, the content of that business.
特定信用情報提供等業務及び特定信用情報提供等業務に付随する業務以外の業務を行うときは、その業務の内容
The following documents must accompany the application referred to in the preceding paragraph:

In the case referred to in the preceding paragraph, if the articles of incorporation, inventory of property, balance sheet, profit and loss statement, or income and expenditure statement, or business report has been created as an electronic or magnetic record, the electronic or magnetic record (limited to one as specified by Order of the Ministry of Economy, Trade and Industry) may accompany the application in lieu of a paper document.
