Article 14-9Amount Specified by Cabinet Order under Article 9-4-3, Paragraph (1) of the Supplementary Provisions of the Act
第十四条の九（法附則第九条の四の三第一項の政令で定める額）
The amount specified by Cabinet Order prescribed in Article 9-4-3, paragraph (1) of the Supplementary Provisions of the Act is, where the month for which the specified premium prescribed in that paragraph (hereinafter referred to as the "specified premium") is paid under that paragraph (hereinafter referred to in this paragraph as the "month paid for") falls in a fiscal year listed in the left-hand column of the following table, the amount obtained by multiplying the amount equivalent to the premium for that month paid for by the rate provided for in the right-hand column of the table for that fiscal year (where that amount has a fraction of less than 10 yen, the fraction is rounded down if it is less than 5 yen, and is calculated as 10 yen if it is 5 yen or more).

法附則第九条の四の三第一項に規定する政令で定める額は、同項の規定により同項に規定する特定保険料（以下「特定保険料」という。）を納付する月（以下この項において「納付対象月」という。）が次の表の上欄に掲げる年度に属する場合において、当該納付対象月に係る保険料に相当する額にそれぞれ同表の下欄に定める率を乗じて得た額（この額に十円未満の端数がある場合においては、その端数金額が五円未満であるときは、これを切り捨て、その端数金額が五円以上であるときは、これを十円として計算する。）とする。

The Minister of Health, Labour and Welfare is to give public notice of the amount provided for in each of the following items that is to be paid in the case listed in that item.
厚生労働大臣は、次の各号に掲げる場合に納付すべき当該各号に定める額を告示するものとする。
In the case of paying the specified premium for each month belonging to a fiscal year listed in the left-hand column of the table in the preceding paragraph (limited to each month in the period within 10 years before the month that includes the day of approval under Article 9-4-3, paragraph (1) of the Supplementary Provisions of the Act): the amount obtained by adding the amount prescribed in the preceding paragraph to the amount equivalent to the premium for each month of the period relating to that payment;
前項の表の上欄に掲げる年度に属する各月（法附則第九条の四の三第一項の承認の日の属する月前十年以内の期間の各月に限る。）について特定保険料を納付する場合 当該納付に係る期間の各月の保険料に相当する額に前項に規定する額を加算した額
