Article 14-14Procedures for the Request under Article 9-4-7, Paragraph (1) of the Supplementary Provisions of the Act
第十四条の十四（法附則第九条の四の七第一項の申出の手続）
An insured person or a person who was an insured person who intends to make the request under Article 9-4-7, paragraph (1) of the Supplementary Provisions of the Act must submit a written request to the Japan Pension Service.
法附則第九条の四の七第一項の申出をしようとする被保険者又は被保険者であつた者は、申出書を機構に提出しなければならない。
