Article 14-22Procedures for Requests under Article 9-4-9, Paragraph (1), Article 9-4-10, Paragraph (1) and Article 9-4-11, Paragraph (1) of the Supplementary Provisions of the Act
第十四条の二十二（法附則第九条の四の九第一項、第九条の四の十第一項及び第九条の四の十一第一項の申出の手続）
An insured person or a person who was an insured person who intends to make a request under Article 9-4-9, paragraph (1), Article 9-4-10, paragraph (1) or Article 9-4-11, paragraph (1) of the Supplementary Provisions of the Act must submit a written request to the Japan Pension Service.
法附則第九条の四の九第一項、第九条の四の十第一項又は第九条の四の十一第一項の申出をしようとする被保険者又は被保険者であつた者は、申出書を機構に提出しなければならない。
