一the employer of the establishing place of business of a surviving employees' pension Fund has failed to submit a notification, or has submitted a false notification, in violation of the provisions of Article 128 of the pre-amendment Employees' Pension Insurance Act that remains in force pursuant to the provisions of Article 5, paragraph (1) of the Supplementary Provisions;

二the employer of the establishing place of business of a surviving employees' pension Fund has failed to submit a notification, or has submitted a false notification, in violation of the provisions of Article 98, paragraph (1) of the pre-amendment Employees' Pension Insurance Act as applied mutatis mutandis in Article 174 of the pre-amendment Employees' Pension Insurance Act that remains in force pursuant to the provisions of Article 5, paragraph (1) of the Supplementary Provisions;

三a member of a surviving employees' pension Fund has failed to submit a notification or has submitted a false notification, or has failed to make a report or has made a false report, in violation of the provisions of Article 98, paragraph (2) of the pre-amendment Employees' Pension Insurance Act as applied mutatis mutandis in Article 174 of the pre-amendment Employees' Pension Insurance Act that remains in force pursuant to the provisions of Article 5, paragraph (1) of the Supplementary Provisions;

四a person obliged to file a notification of death pursuant to the provisions of the Family Register Act (Act No. 224 of 1947) has failed to file a notification in violation of the provisions of the main clause of Article 98, paragraph (4) of the pre-amendment Employees' Pension Insurance Act as applied mutatis mutandis in Article 174 of the pre-amendment Employees' Pension Insurance Act that remains in force pursuant to the provisions of Article 5, paragraph (1) or Article 38, paragraph (1) of the Supplementary Provisions.
