The matters specified by Order of the Ministry of Health, Labour and Welfare under Article 4-2, paragraph (1) of the Act are as follows:
法第四条の二第一項の厚生労働省令で定める事項は、次のとおりとする。

Outline of the business;
事業の概要
Location of the employer;
事業主の所在地
Number of workers pertaining to the business;
事業に係る労働者数
In the case of a business whose period is scheduled in advance (hereinafter referred to as a "business with a definite term"), the scheduled period of the business;
事業の期間が予定される事業（以下「有期事業」という。）にあつては、事業の予定される期間
In the case of a business of construction, alteration, preservation, repair, modification, demolition or dismantling of civil engineering works, buildings or other structures, or of preparation therefor (hereinafter referred to as a "construction business"), the contract amount pertaining to that business (excluding the amount equivalent to consumption tax and local consumption tax (hereinafter referred to as the "amount equivalent to consumption tax, etc."); the same applies hereinafter) (in the case falling under any of the items of Article 13, paragraph (2), meaning the amount calculated as provided in the relevant item; the same applies in Article 6, paragraph (1), item (ii), Article 8, item (ii), Article 34, item (iv) and Article 35, paragraph (1), item (ii)), and the name and the address or location of the orderer;
土木、建築その他の工作物の建設、改造、保存、修理、変更、破壊若しくは解体又はその準備の事業（以下「建設の事業」という。）にあつては、当該事業に係る請負金額（消費税及び地方消費税に相当する額（以下「消費税等相当額」という。）を除く。以下同じ。）（第十三条第二項各号に該当する場合には、当該各号に定めるところにより計算した額をいう。第六条第一項第二号、第八条第二号、第三十四条第四号及び第三十五条第一項第二号において同じ。）並びに発注者の氏名又は名称及び住所又は所在地
In the case of a business of felling of standing trees, the prospective production volume of logs;
立木の伐採の事業にあつては、素材の見込生産量
Where the employer has a corporate number (meaning the corporate number prescribed in Article 2, paragraph (16) of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (Act No. 27 of 2013); the same applies hereinafter), the corporate number of that employer.
事業主が法人番号（行政手続における特定の個人を識別するための番号の利用等に関する法律（平成二十五年法律第二十七号）第二条第十六項に規定する法人番号をいう。以下同じ。）を有する場合には、当該事業主の法人番号
A notification under Article 4-2, paragraph (1) of the Act must be made by submitting it to the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned.
法第四条の二第一項の規定による届出は、所轄労働基準監督署長又は所轄公共職業安定所長に提出することによつて行わなければならない。
When a notification under the preceding paragraph has been submitted, the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned may, if he or she finds it necessary, request the employer to submit a certificate of registered matters or other documents by which the matters listed in the items of paragraph (1) can be confirmed.
所轄労働基準監督署長又は所轄公共職業安定所長は、前項の届出が提出されたときであつて、必要と認めるときには、事業主に対し、登記事項証明書その他の第一項各号に掲げる事項を確認できる書類の提出を求めることができる。

前号の事務であつて、第三項第一号の事業に係るもの及び労働者災害補償保険（以下「労災保険」という。）に係る保険関係のみに係るもののうち、この省令の規定による事務 事業場の所在地を管轄する労働基準監督署長（以下「所轄労働基準監督署長」という。）
