When the Minister of Health, Labour and Welfare has authorized the establishment of a health insurance society, the Minister is to give public notice of the following matters:
厚生労働大臣は、健康保険組合の設立の認可をしたときは、次に掲げる事項を告示するものとする。
the name of the health insurance society;
健康保険組合の名称
the location of its office;
事務所の所在地
the name and location of the establishment (meaning the applicable place of business for which the health insurance society has been established; the same applies hereinafter);
設立事業所（健康保険組合が設立された適用事業所をいう。以下同じ。）の名称及び所在地
the date of the authorization of establishment.
設立の認可の年月日
When the Minister of Health, Labour and Welfare has authorized a change to the constitution or accepted a notification of a change to the constitution, and that change relates to the matters set out in item (i) or (ii) of the preceding paragraph, the Minister is to give public notice of those matters.
健康保険組合
組合員である被保険者の健康保険の事業を行う法人で、全国健康保険協会と並ぶ健康保険の保険者です（健康保険法第四条・第六条・第九条）。適用事業所の事業主、そこに使用される被保険者と任意継続被保険者で組織されます（同法第八条）。常時、政令で定める数以上の被保険者を使用する事業主は単独で、複数の事業主は被保険者の数を合わせてその数以上であれば共同して、設立できます。設立には、その適用事業所ごとに被保険者の二分の一以上の同意を得て規約を作り、厚生労働大臣の認可を受けます（同法第十一条・第十二条）。
