A qualified recipient who has changed the recipient's name or domicile or residence and intends to receive recognition of unemployment or payment of the basic allowance must, on the day of recognition of unemployment or the payment day, submit to the Chief of the competent Public Employment Security Office, accompanied by a driver's license or other document that can prove the fact of the change of name or domicile or residence and by the qualified recipient's certificate (or, where that qualified recipient has been issued a notice of recipient qualification, presenting an Individual Number Card), a qualified recipient's notification of change of name (Form No. 20) in the case of a change of name, or a qualified recipient's notification of change of address (Form No. 20) in the case of a change of domicile or residence.
When the Chief of the competent Public Employment Security Office has received submission of a qualified recipient's notification of change of name or a qualified recipient's notification of change of address, the Chief must make the necessary revision to the qualified recipient's certificate and return it (or, where the person who made that submission has been issued a notice of recipient qualification, enter the necessary matters in the notice of recipient qualification and deliver it).
The provisions of Article 17-2, paragraph (4) and the proviso to Article 22, paragraph (1) apply mutatis mutandis to the case under paragraph (1).
